About this file
This document is a medical imaging interpretation template for chest X-ray TB screening assessments. It provides standardized reporting formats for both negative and abnormal radiographic findings.
For negative/normal films, the template documents a single PA view chest X-ray examination with clinical data evaluating for tuberculosis. The findings section specifies normal parameters including midline trachea, normal heart size, absence of infiltrate, effusion, pneumothorax, acute pathology, atherosclerotic disease, and osseous abnormalities. The impression indicates negative screening for active TB disease with a normal chest X-ray result, requiring nurse review and clearance for the general population. For abnormal films, the template indicates that findings vary according to identified abnormalities and provides a positive screening impression for active TB disease when pulmonary parenchymal findings, pleural findings, and/or mediastinal lymphadenopathy are present that cannot exclude active TB. In such cases, providers should apply the IHSC TB Clinical Guideline screening algorithm using Category A, B, or C classifications to determine further action if necessary.

Text version
Negative/normal film:
EXAM: X-RAY OF THE CHEST
CLINICAL DATA: Evaluate for TB.
TECHNIQUE : Single PA view of the chest.
PRIOR STUDIES: No prior studies submitted.
FINDINGS: No radiographic evidence of TB. The trachea is midline. The heart size is within normal limits. No infiltrate, effusion or pneumothorax. The soft tissues and bony structures demonstrate no acute pathology. No evidence of atherosclerotic disease of the aorta.
No acute osseous abnormality is detected.
IMPRESSION: Negative screening for Active TB disease by radiographic criteria: normal CXR. Requires nurse review; Clear for general population.
Abnormal film:
Findings vary according to abnormality(ies) found.
Impression: POSITIVE screening for Active TB disease by radiographic criteria: pulmonary parenchymal and/or pleural finding(s) and/or mediastinal lymphadenopathy, cannot exclude Active TB; findings as noted above. A provider should apply IHSC TB Clinical Guideline screening algorithm (Category A, B or C) for further action, if necessary.
