About this file

This is a Certificate of Liability Insurance sample document (ACORD 25 form) provided by the Laredo Independent School District in Texas for use by all vendors and contractors engaged with the district. The document outlines mandatory insurance requirements that must be submitted by vendors and contractors prior to contract execution. All vendors and contractors must provide certificates of insurance naming Laredo ISD as the certificate holder and additional insured, with coverage including general liability, auto liability, umbrella/excess liability, and workers' compensation. The certificate must specify the insurance agent or broker providing the certificate, the insurance company writing the policy, policy numbers, and effective dates. Notice of cancellation requirements mandate a minimum of 30 days' notice to the certificate holder, except in cases of non-payment where 10 days' notice is permissible.

The minimum insurance limits required are $1,000,000 for general liability, $1,000,000 for auto liability, $5,000,000 for umbrella excess liability, and statutory limits for workers' compensation. Coverage must be primary and non-contributory with respect to general liability, automobile, and umbrella liability policies. All policies must include a waiver of subrogation, including workers' compensation, and must be endorsed to provide the specified notice of cancellation periods. The certificate holder address is identified as Laredo Independent School District, 900 East Lyons, Laredo, TX 78040. Higher insurance limits may be required depending on the nature and scope of the contract. This sample certificate serves as a template for vendors bidding on Laredo ISD contracts, including the property sale solicitation.
