About this file
This document is a Vendors Certification form from Halifax Health regarding scrutinized companies, focused on compliance with Florida Statutes § 287.135. The certification requires vendors to affirm they are not on specific lists of companies that boycott Israel, have activities in Sudan or Iran, or conduct business operations in Cuba or Syria. Vendors must disclose their status when bidding on or renewing contracts, with different restrictions applying to contracts under and over $1 million.
The document outlines significant legal and contractual consequences for false certifications, including potential civil penalties, attorney's fees, and contract termination. For contracts of any amount, Halifax Health can terminate the agreement if a vendor submits a false certification or is found to be boycotting Israel. For contracts over $1 million, additional grounds for termination include being listed on scrutinized companies lists related to Sudan, Iran, or conducting business in Cuba or Syria. The form requires vendors to provide company details, contact information, and an authorized representative's signature certifying compliance with these statutory requirements.
