About this file

This document is a compilation of vendor eligibility and compliance forms issued by the Tohopekaliga Water Authority (Toho) for potential vendors seeking to do business with the organization. The forms cover various state and federal compliance requirements, including E-Verify registration, public entity crime certifications, scrutinized companies declarations, discrimination vendor checks, anti-trust verifications, common carrier certifications, trench safety compliance, anti-human trafficking attestations, federal suspension and debarment certifications, Byrd Anti-Lobbying Amendment compliance, and drug-free workplace commitments.

The forms require vendors to certify their eligibility by affirming they are not on any prohibited vendor lists, comply with Florida and federal statutes regarding employment, work authorization, and business practices, and meet specific legal standards for contracting with public entities. Vendors must provide detailed certifications regarding their business practices, workforce policies, and legal standing, including commitments to maintaining a drug-free workplace, not employing unauthorized workers, and adhering to anti-lobbying and anti-human trafficking regulations. These comprehensive forms serve as a mechanism for Toho to ensure potential contractors meet strict legal and ethical standards before entering into a business relationship.
