The affidavit must be completed and returned with the vendor's submittal, or submitted within three business days of the County's request upon award, renewal, or extension of any contract with Broward County. The related contract seeks appraisal services for vehicle and equipment damages for Broward County Fleet Services Division and other county agencies, requiring all licensed appraisers to have a minimum of five years' experience in automobile, truck, or equipment appraisals. Vendors must submit their responses electronically through Bonfire BPRO and include the names, qualifications, certifications, and licenses of assigned appraisers in their proposals.

The form requires vendor name, signature, printed name, title, and date, and was dated September 9, 2024. The solicitation allows for a question and answer period where vendors can seek clarification through the Bonfire BPRO platform, and the Office of Economic and Small Business Development requirements are noted as not applicable to this particular solicitation.
