About this file

This is a Conflict of Interest Questionnaire (Form CIQ) required by Texas Local Government Code Chapter 176 for vendors seeking to do business with local governmental entities. The form is mandated for vendors who have a business relationship with a local government entity and must be filed with the records administrator no later than the seventh business day after the vendor becomes aware of facts requiring disclosure. The questionnaire applies to the Midland International Air & Space Port's request for proposals for automated teller machine (ATM) services, which is being re-bid.

The questionnaire requires vendors to disclose any employment or business relationships with local government officers or their family members that result in taxable income exceeding $2,500 during the preceding 12-month period, any gifts with an aggregate value exceeding $100 given to officers or their family members within the preceding 12-month period, or any family relationships with local government officers. Vendors must certify whether they or their family members are receiving or likely to receive taxable income from the local government officer or related entities, and whether officers or their families are receiving or likely to receive income from the vendor. Failure to file a complete and accurate questionnaire constitutes a misdemeanor offense under Texas law. The form must be signed and dated by an authorized representative of the vendor.
