STATEMENT OF WORK
PERFORMANCE WORK STATEMENT (PWS)
Tobyhanna Confined Space Assessment, Tobyhanna Army Depot
Part 1
General Information
1. GENERAL: This is a non-personnel services contract to provide Tobyhanna Confined Space Assessment. The Government shall not exercise any supervision or control over the contract service providers performing the services herein. Such contract service providers shall be accountable solely to the Contractor who, in turn is responsible to the Government.
1.1 Description of Services/Introduction: The contractor shall provide all personnel, equipment, supplies, facilities, transportation, tools, materials, supervision, and other items and non-personnel services necessary to perform Tobyhanna Confined Space Assessment as defined in this Performance Work Statement except for those items specified as government furnished property and services. The contractor shall perform to the standards in this contract.
1.2 Background: Baseline Confined Space assessment was conducted in 2011. A re-assessment survey was completed in 2016.
1.3 Objectives: Tobyhanna Confined Space Assessment to include a detailed survey of all confined spaces and related hazards, to install Permit Required Confined Space labels, signs, or a metal marker at every confined space, provided by Tobyhanna. In addition, each Confined Space location will be affixed with a weather resistant, durable embossed metal tag with a unique identifier and the following contact information “Contact TYAD Safety 570 615- 7027”, to be provided by the Contractor. The unique identifier to follow the already established identification sequence established by previous Confined Space Assessments and our current GIS Confined Space Compliance Survey Reports.
1.4 Scope: Tobyhanna Confined Space Assessment. Services include: 1. Provide instrumentation, equipment and qualified personnel to conduct a detailed survey of confined spaces and related hazards at Tobyhanna Army Depot (TYAD). The survey will be conducted to meet the compliance requirements specified by the U.S. Department of Labor, Occupational Safety and Health Administration (OSHA), as identified in 29 CFR 1910.146. Confined spaces identified within this survey will also be evaluated for their potential to represent a Permit Required Confined Space, as defined by OSHA, utilizing the 29 CFR 1910.146 Appendix A guidance.
2. There are an estimated 400 identified confined spaces at TYAD. This survey will build from a partial list of identified confined spaces to clearly identify and designate with either a sign, label or marker, dependent on location and environment, provided by TYAD Safety and Occupational Health Office (SOHO), all confined space sites. In addition, each Confined Space location will be affixed with a weather resistant, durable embossed metal tag with a unique identifier and the following contact information “Contact TYAD Safety 570 615-7027”, to be provided by the Contractor. The unique identifier to follow the already established identification sequence established by previous Confined Space Assessments and our current GIS Confined Space Compliance Survey Reports.
Additionally the TYAD Regulation 385-10 Confined Space Entry Program and Contractor Safety Standards for TYAD treats all confined spaces as Permit Confined Spaces, as far as permit required to enter any confined space, labeling, training required to enter, written entry plan, and the requirement of air monitoring and ventilation. Any information provided by TYAD personnel will be handled as operationally sensitive information. A baseline report will include all atmospheric and physical hazards for all Permit Required Confined Spaces, to include the following:
• Atmospheric hazards
• Asphyxiating atmospheres
• Flammable atmospheres
• Toxic atmospheres
• Burn hazards
• Heat stress hazards
• Mechanical hazards
• Engulfment hazards
• Physical hazards (falls, debris, slipping hazards)
• Electrocution
• Danger of unexpected movement of machinery
• Noise hazards.
All confined space work will be accomplished in compliance with TYAD Regulation 385-10 Confined Space Entry Program.
3. Information will be provided to the TYAD SOHO beginning on the second Monday following award of the contract. The information will consist of a listing for each confined space along with the hazards identified and recommended listing as a Permit Required or Non-Permit Required Confined Space. Listings for Permit Required Confined Spaces will include physical and atmospheric hazards associated with each of the identified locations, to include additional hazards listed in paragraph 2.
4. The contractor will develop and publish, in print (hardcopy Four Copies minimum), and in an electronic format agreeable to the TYAD SOHO, the listing of every confined space, its identified hazards, and the recommendation of whether it should be treated as a Permit Required or Non-Permit Required Confined Space. The survey and the compilation of accumulated data shall be completed within a period of twelve months (12). At the completion of the contract, all information (electronic and hard copy) will be turned over to the TYAD SOHO. The contractor shall not retain any electronic nor hard copy for their own use or files.
1.5 Period of Performance: The period of performance shall be for not greater than twelve (12) months.
1.6 General Information
1.6.1 Quality Control: The contractor shall develop and maintain an effective quality control program to ensure services are performed in accordance with this PWS. The contractor shall develop and implement procedures to identify, prevent, and ensure non-recurrence of defective services. The contractor’s quality control program is the means by which he assures himself that his work complies with the requirement of the contract. The QCP is to be delivered within 30 days after contract award. Three copies of a comprehensive written QCP shall be submitted to the COR within 5 working days when changes are made thereafter. After acceptance of the quality control plan the contractor shall receive the contracting officer’s acceptance in writing of any proposed change to his QC system.
1.6.2 Quality Assurance: The government shall evaluate the contractor’s performance under this contract in accordance with the Quality Assurance Surveillance Plan. This plan is primarily focused on what the Government must do to ensure that the contractor has performed in accordance with the performance standards. It defines how the performance standards will be applied, the frequency of surveillance, and the minimum acceptable defect rate(s).
1.6.3 Recognized Holidays: Contractor is not under obligation to perform confined space assessment activities during any of the following recognized Holidays:

1.6.4 Hours of Operation: The contractor is responsible for conducting business, between the hours of 7am to 4pm Monday thru Friday except Federal holidays or when the Government facility is closed due to local or national emergencies, administrative closings, or similar Government directed facility closings. For other than firm fixed price contracts, the contractor will not be reimbursed when the government facility is closed for the above reasons.
The Contractor must at all times maintain an adequate workforce for the uninterrupted performance of all tasks defined within this PWS when the Government facility is not closed for the above reasons. When hiring personnel, the Contractor shall keep in mind that the stability and continuity of the workforce are essential.
1.6.5 Place of Performance: The work to be performed under this contract will be performed at Tobyhanna Army Depot.

All persons seeking entrance to TYAD shall submit to and comply with all security standards and requirements in force at the time such persons are seeking entry. All contractors, regardless of resident status or citizenship, will be subject to vehicle search and intense in-processing by TYAD security personnel prior to being granted access to TYAD. This security screening process may be time consuming and access may be delayed or denied. The contractor shall ensure ELTY Form 648-C is completed for all contractor and subcontractor personnel requiring depot access to include warranty services. The TYAD point of contact (POC) will provide ELTY Form 648-C, Request Access to Tobyhanna Army Depot, to the contractor/vendor at least ten days prior to the expected visit date for completion. All of the required fields on the form shall be complete and accurate by the contractor/vendor for timely processing. This requirement is inclusive of on-site supervisory or managerial personnel and sub-contractor personnel that the Contractor anticipates will be performing work or visiting on-site. This security screening does not relieve the contractor of any responsibilities to conduct thorough pre-employment background checks and drug screening. Contractor workers will not be granted access to the work site until security screening is completed and access is approved. Any contractor personnel on-site who fail screening will not be permitted further access to TYAD. See “Access and General Protection/Security Policy and Procedures” below.
