Questions and Clarifications
| # | Identifier | Technical, Cost, or Business Management | RFP Section and Page Reference | Question or Clarification Request | Offeror Recommendation (if any) | Answers | Results in a Change to the RFP |
| 1 | Solicitation SF33 | Past Performance | b.10 page 29 | Will executive level past performance be accepted for a new business entity? Furthermore is the government evaluating the past performance of both prime and subcontractors equally? | Offeror recommends allowing executive level experience as past performance for new businesses as well as allowing equal evaluation of subcontractors past performance. | See section L.6.3.3, Factor 3: Past Performance, for the instructions related to the past performance evaluation criteria, along with Attachment 3 to the RFP. | No |
| 2 | RFP | Business Management Proposal | L.6.5.3 page 86 | To clarify the government would like the entire SAM printout included within the Business Management Proposal and not just a compliant statement affirming SAM is up to date? "As part of the Business Management Proposal, offerors shall submit all information required under Section K of this solicitation, to include, FAR 52.204-7 (System for Award Management) and FAR 52.204-8 (Annual Representations and Certifications) a “full” printout of Reps and Certs documentation from the System for Award Management (SAM) database." | A compliant statement affirming your registraiton to SAM is up to date will be sufficient. | No | |
| 3 | RFP | Technical Proposal | L.6.3.3 Factor 3: Past Performance Page 83 | To clarify is there a maximum amount of past performance entries allowable? "The offer must complete Part I of the attachment 3, Past Performance Questionnaire document, providing reference point of contact and related information on at least two prior contracts for similar requirements as described in this solicitation for the prime, joint vendor partner and major subcontractors." | There is no maximum amount of past performance entries allowable, but the Government would recommend no more than 5. | No | |
| 4 | RFP | Technical Proposal | L.6.3.3 Factor 3: Past Performance Page 83 | Should Past Performance Questionnaires being extracted and included as tables within the Technical Proposal or would the government prefer these files be left in excel format as separate individual attachments outside of the Technical Proposal? | The Government's preference would be to have the files submitted in excel format as separate individual attachments to the Technical Proposal. | No | |
| 5 | NA | General | General | Can the government advise if these services are currently being supported and managed by the government? If so, why is the government procuring the services through a federal contract? | The services are currently being provided through contract. | No | |
| 6 | NA | General | General | Are all positions currently being filled with no vacancies? If not, please list the positions that are currently not being fullfilled. | Yes, the current contract is fully staffed. | No | |
| 7 | NA | General | General | If this contract is currently incumbered please provide the current contract number or numbers and contractors name/names. | The services are currently being provided by Johns Hopkins Healthcare LLC under contract 19AQMM20C0144. | No | |
| 8 | C.5: QASP (Quality Assurance) | Technical | p.12 | Will the attached (developed as part of current FASTC clinic with (name withheld) suffice for this QASP requirement? | Please refer to the solicitation for instructions. | No | |
| 9 | H5: Subcontracting | General | p.28 / Subcontracting Plan Attachment | As part of this contract, (company name redacted) subcontracts a (company name) entity through the Department of Occupational and Environmental Medicine (DOEM), but they do not qualify as a small business because they are not a separate legal entity, they are part of the university. The DOEM team will manage the day to day operations and oversight of the clinic. Given that explanation, we would like to confirm that there is no need to fill out the Subcontractor Plan attachment. | Offerors considered an Other Than Small Business under the NAICS Code size standard, must submit a Subcontracting Plan with their proposal response. Please reference NAICS 621111 for size standards. | No | |
| 10 | K.2.1: Representations, Certifications & Statement of Offerors | General | p.45 | Does (vendor name redacted) need to submit anything in Section K based on your knowledge of our initial contract? We interpret that we do not, but are not sure. | All offerors are required to complete the applicable Section K reps and certs. | No | |
| 11 | L.6.2.CAGE Code | General | p.80 | We are unclear on what Cage Code to use as part of the proposal. Can you clarify? Our previous Cage Code was (redacted). | Vendors are required to submit their assigned cage code following the applicable procedures. | No | |
| 12 | L.6.3 Technical Proposal Requirements | Technical | p.81 | Can you provide an example of the "compliance checklist" referred to in L.6.3 c)? If we list the items included in the RFP proposal and confirm on which pages of the proposal they are addressed, does that suffice? | Listing the items required to be submitted in the RFP proposal and identifying the pages and sections of the proposal where they are addressed will suffice. | No | |
| 13 | L.6.5.2 Business Systems, Joint Venture, Policies | Technical | p.85 | Can you please confirm if we need to provide a Busines Management Proposal as part of this RFP? Given that we would be providing a "service" to the FASTC base, and will be outlining the scope of work/services in the Technical Proposal, we assume that the Business Management Proposal is out-of-scope for this RFP. Please confirm. | Please follow the instructions in section L.6.5. | No | |
| 14 | Technical | L.6.3.3, Page 83 | L.6.3.3 states that the offeror must provide "reference point of contact and related information on at least two prior contracts for similar requirements as described in this solicitation for the prime, joint vendor partner and major subcontractors." Does the Government mean that a minimum of two references must be provided per teaming partner or a minimum of two references for the entire team? | A minimum of two references are required for the entire team. | No | ||
| 15 | Technical | L.6.3.2 (b), Page 82; 652.219-70 | Given the scale of the services requested, the Department of State Subcontracting Goals in 652..219-70 on page 31 of the Solicitation document are not practical for large business offerors. Will the Government consider eliminating these goals for this procurement? | Offerors are required to propose their subcontracting goals, which should comply with the DOS subcontracting goals. If an offeror is not able to meet the DOS subcontracting goals, an explanation as to why the goals cannot be achieved must be provided. | No | ||
| 16 | Technical | L.6.3.2(b), Pages 82-83 | The amount of items required for large business concerns to submit as part of the Subcontracting Plan appear to be unduly restrictive to large business offerors. Will the Government consider eliminating the requirements of Subfactor 2.1 and have large business offerors only submit a completed Attachment 4 to satisfy the subcontracting plan requirement for this solicitation? | Recommend that the specified requirements in Subfactor 2.1 be replaced with the completion of the subcontracting plan provided in Attachment 4. | Subfactor 2.1 was included at the request of the Small Business Administration (SBA), and will not be removed. All other than small businesses are required to address L.6.3.2 Factor 2: Levels of Service by Service Line as per b) Sub-Factor 2.1: Subcontracting Plan accordingly. |

| 17 | Cost | C.3.1, Page 7 | C.3.1 states that the contractor will support the "initial set-up and medical furnishings/supplies of the two patient treatment rooms…". As it is our understanding that these services currently exist, will the Contractor be responsible for pricing all of the medical furnishings for these treatment rooms or is there existing furnishings that can be transferred to the successful Contractor? | The two treatment rooms are currently furnished to provide adequate care to meet the contract objectives and will remain on site and become government furnished equipment. In the future, it will be necessary to purchase additional supplies and equipment as needed and these costs should be projected on the Other Direct Costs (ODC) lines of the pricing table. | No | ||
| 18 | Technical | C.2(d), Page 7 | Is there a telehealth platform currently being used/preferred by the Government? If so, is the contractor able to access this system? | There is not currently a platform for telehealth. | No | ||
| 19 | Technical | L.6.3.3, Page 83 | Can executive past performance be submitted as a past performance reference? | See response to question #1 | No | ||
| 20 | Cost | C.3.1, Page 7 | Will the Government provide a list of supplies and medications required? | Offeror should provide their list of preferred supplies and medications to meet the requirements of a occupational health clinic. | No | ||
| 21 | Cost | C3.1 Page 7 | Are the supplies Cost reimbursement or does the vendor need to estimate pricing and take risk on overall cost? | Section B.2(d) discusses how supplies and equipment is to be priced as other direct costs (ODC). ODCs will be reimbursed and will not include any fee/profit. | No | ||
| 22 | General | General | Is there currently an incumbent contract that is fulfilling these services? If so, would you be able to provide me with the contract ID number and the vendor name? | ||||
| Or would this be considered a new requirement? | See response to question #7 | No | |||||
| 23 | C3 number 3 of the Statement of Objectives | C3 number 3 of the Statement of Objectives, | In section C3 number 3 of the Statement of Objectives, the Solicitation states: "Provide adequate staffing to support the clinic operations 60 hours per week from 0600-1800 Monday - Friday." |
However, the next paragraph in section C3 number 3 reads as follows:
"Current effective staffing levels is:
- 1 FTE medical provider (Nurse Practitioner or Physician's Assistant)
- 1 FTE Certified Occupational Health Nurse
- 1 FTE Medical Technician
- .05 FTE Physician"
The latent ambiguity that we would respectfully request to clarify is as follows:
Is 1 FTE Nurse Practitioner or Physician's Assistant, 1 FTE Certified Occupational Health Nurse, and 1 FTE Medical Technician required to be provided--with each FTE comprising an established 1920 hours of service per FTE or Is one individual Nurse Practitioner or Physician's Assistant, one individual Certified Occupational Health Nurse, and one individual Medical Technician required to be furnished during all hours of clinic operation,(i.e. from 0600-1800 Monday-Friday)?
If the former answer applies, then a total of 5760 hours would be required for 1 FTE Nurse Practitioner or Physician's Assistant (1920 hours), 1 FTE Certified Occupational Health Nurse (1920 hours), and 1 FTE Medical Technician (1920 hours).
If the latter staffing option is required then a total of 8,640 hours would be required for 1 Nurse Practitioner or Physician's Assistant (2880 hours), 1 Certified Occupational Health Nurse (2880 hours), and 1 Medical Technician (2880 hours). The current effective staffing level provides the 1 FTE medical provider (Nurse Practitioner or Physician's Assistant), 1 FTE Certified Occupational Health Nurse, and 1 FTE Medical Technician with 1920 hours of service for each FTE. The three positions have staggard office hours to ensure adequate coverage from 0600-1800. The .05 FTE Physician is for program virtual oversight and not on site except for rare visits. No
