About this file
This is a Request for Bid (RFB) issued by the University of Wisconsin-Madison Purchasing Services Department on behalf of the Department of Pediatrics for the procurement of 13 laptop-based Pulmonary Function Testing (PFT) systems with spirometry capabilities and one reader station. The equipment will be distributed to UW-Madison and 12 participating research sites across the continental United States for use in the Children's Respiratory and Environmental Workgroup (CREW) study focused on identifying types of childhood asthma and environmental influences on disease development. The systems must be 100% compatible with existing Inner City Asthma Consortium (ICAC) equipment, including the SentrySuite reader station, Rho Inc. web-based database system, and MS Pneumo spirometry systems. All equipment must include heated pneumotach-based flow sensors meeting ATS/ERS standards, recommended laptop configurations, comprehensive software with GLI, ECCS, and NHANES prediction sets, and complete supplies and accessories for full functionality. Written questions regarding the bid were due by October 6, 2017, with bids due on October 13, 2017, at 2:00 PM CDT. The RFB does not specify oral presentations, product demonstrations, or site visits as mandatory evaluation components, though the University reserves the right to require them at its discretion. Bids must be submitted in one hard copy and one digital copy (CD/DVD/Flash Drive) to UW-Madison Purchasing Services at 21 N. Park Street, Suite 6101, Madison, WI 53715-1218, with late bids automatically rejected.
Pricing must be submitted as unit list price, unit net price, and extended net total for each system on the Itemized Bid List. All bids must include F.O.B. Destination pricing with delivery charges and risk of loss borne by the contractor, including all packing, freight, insurance, installation/operation manuals, and delivery to interior/ground floor locations. Custom charges and import fees should not be included in the bid price, with the University arranging payment through its designated customs broker. Award will be made to the lowest extended net total bid judged to be in the University's best interest, with potential consideration of life cycle costs including energy efficiency, maintenance fees, warranties, and disposal costs. The University does not guarantee to purchase any specific quantity or dollar amount, and all bid prices are firm for 60 days from the date of bid opening. Bidders must meet mandatory qualifications including three years of business experience in providing the requested items, authorization as an original manufacturer or authorized distributor with service and repair capabilities, submission of three client references, and certification of non-debarment status. Standard contract terms include Net 30 payment terms upon acceptance and testing, one-year manufacturer warranty, unlimited call-back maintenance service during normal business hours, F.O.B. Destination delivery, and compliance with Wisconsin and federal regulations including nondiscrimination, debarment, and affirmative action requirements for contracts exceeding $50,000.
