About this file
This document contains the required bid attachments for IFB 161201, a Spray Injection Pothole Patching contract administered by the Virginia Department of Transportation (VDOT) across multiple counties in Virginia. The solicitation requires bidders to provide two mobile self-contained spray injection systems to perform pothole patching services in Greensville, Isle of Wight, Southampton, and Sussex Counties. The attachments establish mandatory submission requirements including State Corporation Commission registration information, small business subcontracting plans, vendor qualification and equipment inventory certifications, subcontractor approval documentation, normal and emergency contact information, contractor license requirements, and compliance with Virginia Code Red Ozone Days restrictions for specified jurisdictions. All attachments must be completed and returned with the bid response to avoid rejection or a finding of non-responsiveness.
The solicitation establishes a goal that over 42% of Commonwealth purchases be made from small businesses, requiring all bidders to complete a Small Business Subcontracting Plan identifying DSBSD-certified small business utilization. Bidders must declare whether their firm is DSBSD-certified as a small or micro business or detail specific subcontracting plans for small business participation. Equipment verification requirements mandate that owned equipment be inspected in-person prior to award, rented or leased equipment requires signed rental agreements confirming availability for the contract duration, subcontracted equipment requires Subcontractor Approval Request Forms, and purchased equipment requires proof of purchase with confirmed delivery dates. Prevailing wage determinations are established by the Virginia Department of Labor and Industry for each county, with rates varying by classification and location. Contractor license requirements apply based on contract value thresholds, and all contractors and subcontractors must maintain required licenses throughout the contract term and immediately notify VDOT of any license revocation.
