Statement of Work

LEASE OF NEW MULTI-FUNCTIONAL DEVICE (MFD) AND MAINTENANCE AGREEMENT

Part 1 - General Information

1.1 Purpose: Lease a new multi-functional device (MFD) and maintenance agreement for a total of five years (60-month term) for the Department of Labor (DOL), Office of Workers’ Compensation Programs (OWCP), – Division of Energy Employees ‘Occupational Illness Compensation, (DEEOIC) – National Field Operations (NFO), 400 West St., Suite 722, Jacksonville, FL 32202.

1.2 Background: The OWCP needs to replace the existing outdated MFD which is at the end of its lifecycle, with a new and more current model.

1.3 Objectives: To acquire a new MFD to handle a volume of printing of at least 600,000 pages per month, and a maintenance agreement to cover a period of five years.

1.4 Scope: The Contractor shall furnish and install a new MFD and ancillary products with the specifications set forth below.

These device specifications represent the base configuration that should be applied to the MFD under this SOW. The Contractor shall indicate as part of its offerings all of the optional features that can be added to each device (e.g. additional paper trays, etc.)

· Comply with Department of Labor security requirements

· MFD must have a Voluntary Product Accessibility Template (VPAT) and additional 508 compliance documentation available for review.

The Contractor shall also provide a maintenance agreement to cover a five-year period of performance as specified below.

1.5 Period of Performance: The delivery should be within 90 days of award. A modification will be processed to reflect the 12-month lease term based on the delivery date. There will be one (1) 12-month base period and four (4) 12-month options based on the delivery date.

1.6 General:

1.6.1 Type of Contract: Firm fixed price award.

1.6.2 Recognized Holidays:

The contractor is not required to perform services on these days Holidays:

Presidential Inauguration Day

Most Federal offices in the District of Columbia/Maryland/Virginia (DMV) metropolitan area are closed on the day a President is inaugurated on January 20th for each fourth year after 1965 (see 5 U.S.C. 6103(c)).

1.6.3 Hours of Performance: The Contractor is responsible for conducting business, between the hours of 9 a.m. and 5 p.m., Monday thru Friday except recognized holidays listed in “Recognized Holidays” section above or when the Government facility is closed due to local or national emergencies, administrative closings, or similar Government directed facility closings. The Contractor must at all times maintain an adequate workforce for the uninterrupted performance of all maintenance and problem resolution defined within this SOW when the Government facility is not closed for the stated reasons.

1.6.4 Place of Performance: The work to be performed under this contract will be performed at the Office of Workers’ Compensation Programs (OWCP), Division of Energy Employees ‘Occupational Illness Compensation (DEEOIC), 400 West Bay St., Suite 722, Jacksonville, Florida 32202.

The Contractor will contact Tina Saunders at 904-947-8022, to schedule delivery. Delivery shall occur between the hours of 9:00 a.m.

and 3:00 p.m., Monday through Friday, after coordination.

1.6.5 Security: Contractors shall be escorted within the designated DOL location. Contractors shall furnish identification (I.D.) in the form of a driver’s license or other State issued I.D.; or Federal identification such as a passport or “Lawful Card” as provided by the U.S. Citizenship and Immigration Services (USCIS) if required.

1.6.6 Physical Security: The Contractor shall be responsible for safeguarding all government equipment, information and property provided for Contractor use. At the close of each work period, government facilities, equipment, and materials shall be secured.

1.6.7 Contracting Officer’s Representative (COR): The COR will be identified by separate letter. The COR monitors all technical aspects of the contract and assists in contract administration The COR is authorized to perform the following functions:

assure that the Contractor performs the technical requirements of the contract; perform inspections necessary in connection with contract performance; maintain written and oral communications with the Contractor concerning technical aspects of the contract;

issue written interpretations of technical requirements, including Government drawings, designs, specifications: monitor Contractor's performance and notifies both the Contracting Officer and Contractor of any deficiencies; coordinate availability of government furnished property, and provide site entry of Contractor personnel to the PSB facility and monitoring of activities while on-site for maintenance and support. A modification is sent to the Contractor, naming the COR; the COR is not authorized to change any of the terms and conditions of the resulting order.

1.6.8 Data Rights: N/A

1.6.9 Organizational Conflict of Interest: N/A

1.6.10 Problem Resolution: The Contractor shall provide a mechanism to resolve issues, either invoice or service. A point of contact, including name and telephone number, is to be provided to resolve any issue. In the event the point of contact changes, then the Contractor shall provide updated information within five (5) business days.

1.6.11 Local Area Network (LAN): Network personnel at the OWCP location request notification and involvement in services that require: 1) IP setting changes, 2) adding drivers to a PC/Server, or 3) software updates to the server. If contract employees determine a service request fall into a category requiring LAN assistance, the employee will notify the OWCP employee for these services to be arranged.

Part 2 - Definitions and Acronyms

2.0 Definitions and Acronyms:

2.1 Definitions:

2.1.1 Contractor. A supplier or Contractor awarded a contract to provide specific supplies or service to the government. The term used in this contract refers to the prime.

2.1.2 Contracting Officer. A person with authority to enter into, administer, and or terminate contracts, and make related determinations and findings on behalf of the government. Note: The only individual who can legally bind the government.

2.1.3 Contracting Officer’s Representative (COR). An employee of the U.S. Government appointed by the contracting officer to administer the contract. Such appointment shall be in writing and shall state the scope of authority and limitations. This individual has authority to provide technical direction to the Contractor as long as that direction is within the scope of the contract, does not constitute a change, and has no funding implications. This individual does NOT have authority to change the terms and conditions of the contract.

2.1.4 Physical Security. Actions that prevent the loss or damage of Government property.

2.1.5 Work Day. The number of hours per day the Contractor provides services in accordance with the contract.

2.1.6 Work Week. Monday through Friday, unless specified otherwise.

Part 3 - Government Furnished Property, Equipment, and Services

3.1 General: The government shall provide, the facilities, equipment, materials, and/or services listed below.

3.2 Services:

3.2.1 Utilities. All utilities in the facility will be available for the contractor’s use in performance of duties outlined in this SOW.

The contractor shall be responsible for operating under conditions that preclude the waste of utilities.

Part 4 - Contractor Furnished Items and Services

4.1 General: Except for those items specifically stated to be government furnished in Part 3, the contractor shall furnish everything required to perform this requirement.

Part 5 - Specific Tasks and Deliverables

5.1 Multi-Function Device.

5.1.1 Specifications: The vendor will provide a new multi-function device meeting, at a minimum, the following salient characteristics:

These device specifications represent the base configuration that should be applied to the MFD under this statement of work (SOW).

The Contractor shall indicate as part of its offerings all of the optional features that can be added to each device (e.g. additional paper trays, etc.)

· Ability to network print, network scan, copy, and fax

· Support resolution minimums of 600 X 600 dpi print resolution; max of 2,400 X 4,800 dpi print resolution

· Ability to connect a variety of network operating platforms

· Ability to handle various paper sizes and weights (up to 110 lb. index) to include but not limited to:

o letter (8.5” x11”) o legal (8.5” x 14”) o pamphlet (11” x “17) optional

· Automatic document feeder (minimum 50-sheet capability and ability to handle letter, legal, and tabloid-sized originals)

· Secure printed document storage to require a password to print (user prints to MFD and sets password which must be entered on MF to print the document)

· Minimum of three-drawer paper storage.

o One standard size (capacity for at least 550 sheets) o One large (capacity for at least 2000 sheets) o One legal (capacity for at least 550 sheets) stapling finisher, hole punch, Fax o 8,000 page capacity

· Ability to handle a volume of printing of at least 600,000 pages per month.

· Automatic duplex. (Fully automatic means that no operator handling of throughput or originals is required to obtain two-sided copies from one- or two-sided originals once the originals are inserted into the feeder and the start button is pressed)

· Keypad and/or touchscreen displays

· Self-diagnostic system that indicates at a minimum, the following conditions:

o maintenance needs o toner needs o paper needs o paper jams

· Ability to be reformatted and restored to a factory condition

· Printer Control Language (PCL) and Post-Script (PS) printer language

· Newly manufactured MFD must be in current production at the time provided to the ordering activity. Current production means the MFD is being manufactured as new for the United States Government market; no device provided under this SOW shall be a device that has been converted to new model status or the like. (gray market items shall be prohibited)

· Various finishing options should include but are not limited to options for:

o Sort o Collate o Staple (e.g. 50 sheet, 100 sheet, etc.)

o Hole-punch (2 and 3 holes) o Various fold options o MFD Specification Requirements

5.1.2 Delivery Time: The equipment will be delivered within 90 days of award of this order.

5.1.3 Delivery Location: The equipment will be delivered to:

Department of Labor/OWCP, Division of Energy Employees ‘Occupational Illness Compensation 400 West Bay St., Suite 722 Jacksonville, FL 32202

5.1.4 Shipping Terms: FOB-DEST

5.2 Installation and Maintenance: The vendor will provide at a minimum, the following services:

5.2.1 Installation: The contractor shall provide all personnel, equipment, tools, materials, and other items and non-personal services necessary to install the new MFD.

5.2.2 Maintenance: The printer and its associated components shall be maintained under contract conditions and shall perform according to the Contractor’s equipment standards. The contractor must be an authorized service provider and must be able to provide documentation of the certification. The Contractor shall provide full maintenance and repair services that include both preventive and emergency repair with genuine OEM parts and OEM trained technicians.

Repair technicians must arrive at the OWCP location within 24 hours of repair calls. Contractor will include parts, consumable supplies, delivery, and labor, as applicable.

5.2.3 Consumables: Contract shall provide consumables as part of the maintenance agreement for items such as Toner, Waste Container, and parts for the contract duration. Upon re-order by designated OWCP personnel, requested consumables should be delivered to the aforesaid OWCP location within three (3) business days of ordering. Consumables do not include staples or paper.

(a)(1) The North American Industry Classification System (NAICS) code for this acquisition is [insert NAICS code].

(2) The small business size standard is [insert size standard].

(3) The small business size standard for a concern that submits an offer, other than on a construction or service acquisition, but proposes to furnish an end item that it did not itself manufacture, process, or produce is 500 employees, or 150 employees for information technology value-added resellers under NAICS code 541519, if the acquisition-

(d) of this provision instead of completing the corresponding individual representations and certifications in the solicitation. The Offeror shall indicate which option applies by checking one of the following boxes:

Reasonable inquiry has the meaning provided in the clause 52.204-25, Prohibition on Contracting for Certain Telecommunications and Video Surveillance Services or Equipment.

Restricted business operations means business operations in Sudan that include power production activities, mineral extraction activities, oil-related activities, or the production of military equipment, as those terms are defined in the Sudan Accountability and Divestment Act of 2007 (Pub. L. 110-174). Restricted business operations do not include business operations that the person (as that term is defined in Section 2 of the Sudan Accountability and Divestment Act of 2007) conducting the business can demonstrate-

(1) Are conducted under contract directly and exclusively with the regional government of southern Sudan;

Service-disabled veteran-owned small business (SDVOSB) concern eligible under the SDVOSB Program means an SDVOSB concern that-

(1) Effective January 1, 2024, is designated in the System for Award Management (SAM) as certified by the Small Business Administration (SBA) in accordance with 13 CFR 128.300; or

Service-disabled veteran-owned small business (SDVOSB) Program means a program that authorizes contracting officers to limit competition, including award on a sole-source basis, to SDVOSB concerns eligible under the SDVOSB Program.

Small business concern-(1) Means a concern, including its affiliates, that is independently owned and operated, not dominant in its field of operation, and qualified as a small business under the criteria in 13 CFR part 121 and size standards in this solicitation.
