1. This quality assurance surveillance plan (QASP) is pursuant to the requirements listed in the PWS entitled

Corrections/Detention Officer (CDO) Services. This plan:

a. Sets forth the procedures and guidelines the USMS will use to ensure the required performance standards or service levels are achieved by the Contractor.

USMS Eastern District of Washington’s Armed Guard/Transport Services 11 | P a g e

b. Describes the methods the USMS will use to monitor performance and to identify the required documentation and the resources to be employed.

c. Defines the roles and responsibilities, identifies the performance standards, defines the methodologies used to monitor and evaluate the Contractor’s performance, describes quality assurance documentation requirements, and describes the analysis of quality assurance monitoring results.

d. Enables the government to take advantage of the Contractor’s Quality Control Plan (QCP) when evaluating whether the Contractor is meeting the performance standards/quality levels identified in the PWS and to ensure the government pays only for the level of services received.

B. Performance Management Approach

1. The PWS structures the acquisition around “what” service or quality level is required, as opposed to “how” the Contractor should perform the work (i.e., results, not compliance).

2. The QASP defines the performance management approach taken by the USMS to monitor and manage the Contractor’s performance to ensure the expected outcomes or performance standards communicated in the PWS are achieved.

C. Performance Management Strategy

1. The Contractor is responsible for the quality of all work performed.

a. The Contractor measures quality through its own QCP.

b. Quality control includes all work performed under this contract regardless of whether the work is performed by Contractor’s employees or by subcontractors.

2. The Contractor shall provide a QCP to the CO and the District COR for acceptance at least five (5) days prior to providing service to ensure policies, SOPs, processes, and PWS standards are followed and are sufficient.

a. The District COR shall notify the Contractor of acceptance or required modifications to the QCP within five (5) business days.

b. The Contractor shall make appropriate modifications and obtain final acceptance of the QCP by the District COR within five (5) business days of notification of required changes.

c. After final acceptance, the Contractor shall implement the QCP.

d. At a minimum, the QCP shall include:

(1) Staffing and procedures for self-inspecting the quality, timeliness, responsiveness, customer satisfaction, and other performance requirements in the PWS.

(2) A description of the methods to be used for identifying and preventing deficiencies in the quality of service provided.

(3) A description of records management and retention plans

(4) Processes and procedures to ensure:

(a) CDOs adhere to policies and procedures

(b) CDOs are uniformed and armed when required

(c) Proper restraining devices are present and being utilized correctly

(d) Vehicle and Hospital Logs are complete

(e) Forms are completed as required by the District COR and the contract

D. Roles and Responsibilities

1. Contracting Officer (CO)

a. The CO is responsible for monitoring contract compliance, contract administration, and cost control and for resolving any differences between the observations documented by the District COR and the Contractor.

USMS Eastern District of Washington’s Armed Guard/Transport Services 12 | P a g e

b. The CO will designate one full-time District COR as the government authority for performance management. The number of additional representatives serving as technical inspectors will depend on the volume of service requested and the Contractor’s performance.

2. Contracting Officer’s Representative (COR)

a. The District COR is designated in writing by the CO to act as his or her authorized representative to assist in administering the contract.

b. The District COR is responsible for technical administration of the provided service and ensures proper government surveillance of the Contractor’s performance.

c. The District COR is not empowered to make any contractual commitments or to authorize any contractual changes on the government’s behalf. Any changes the Contractor deems may affect contract price, terms, or conditions shall be referred to the CO for action.

d. The District COR will have the responsibility for completing QA monitoring forms used to document the inspection and evaluation of the Contractor’s work performance.

E. Methodologies to Monitor Performance

1. Government surveillance may occur under the inspection of services clause for any service relating to the contract.

2. If the Government performs inspections on the premises of the Contractor or a subcontractor, the Contractor shall furnish, and shall require subcontractors to furnish, at no cost, all reasonable facilities and assistance for convenient performance of these duties.

3. Surveillance Techniques

a. In an effort to minimize the performance management burden, simplified surveillance methods will be used by the government to evaluate Contractor performance when appropriate.

b. The primary methods of surveillance are:

(1) Random Monitoring – The District COR or designated USMS inspector will randomly inspect items in the Performance Summary Requirement (PSR) table below.

(2) Periodic Inspection – The District COR will typically perform monthly periodic inspections of PSR items.

(3) Annual Inspection – Each year, the District COR will review generated documentation (e.g., reports, logs, published plans and procedures, certifications, qualifications, personnel files, performance issue resolutions) and provide a report to the Contractor and Contractor Performance Assessment Reporting System (CPARS).

(4) Complaint – The District COR will investigate and document all complaints using the Quality Assurance Monitoring form below.

2. The District COR will retain a copy of all completed QA Monitoring forms.

H. Analysis of Quality Assurance Assessment

1. Determining Performance

a. The District COR will use the monitoring methods previously cited to determine whether the performance standards have been met.

b. If the Contractor has not met the minimum requirements, it may be asked to develop a Corrective Action Plan to show how and by what date it intends to bring performance up to the required levels.

2. Reporting

a. The District COR will prepare an annual written report for the CO summarizing the overall results of the quality assurance surveillance of the Contractor’s performance.

(1) This report, and associated quality assurance monitoring forms, will become part of the QA documentation.

PART II - EVALUATION FACTORS FOR AWARD:

Federal Acquisition Regulation, Part 13 Simplified Acquisition Procedures are applicable to the RFP with using FAR 52.212-2 Evaluation-Commercial Items for the evaluation criteria. The Government will award a contract resulting from this solicitation to the responsible offeror whose offer, conforming to the solicitation will be most advantageous to the Government, price and other factors considered.

The following factors shall be used to evaluate offers:

1. Technical Capability

2. Past Performance

3. Price

Evaluation may be made with or without discussions with the offeror. Technical Capability and Past Performance, when combined, are considered to be significantly more important than Price.

1) TECHNICAL CAPABILITY – Technical Capability must be presented to the Government in written format. The specific technical evaluation section listed below must be clearly and fully addressed in the proposal. The response to the section listed below must contain an adequate description of the services being offered in sufficient detail for the Technical Evaluation Board (TEB) to determine that the offeror can meet the requirements of the solicitation.

The Technical Capability criteria below will be used to comparatively evaluate each offer.

1. Technical Expertise: A narrative describing your understanding of this requirement and a description of your technical expertise for meeting the requirements of this solicitation. The narrative should be limited to no more than 25 pages, and should, at a minimum include:

• Demonstrate understanding of the overall and specific requirements in the Performance Work Statement (PWS), specifically Section III, Requirements;

convey the company’s capabilities; provide in detail, the plans and methods for accomplishing safe and secure custody of USMS prisoners while transporting prisoners.

• The narrative shall be clear, concise, and include sufficient detail for effective evaluation and substantiation of all information. The narrative should not rephrase or restate the Government’s requirements, but rather shall provide rationale to address how the offeror intends to meet the requirements.

• Provide License to certify the firm can operate guard services in the State of

Washington

• Shall confirm the company is capable to submit all the deliverables in the PWS Section III, Q. Deliverables, within the specified timelines.

• Shall confirm the company is capable to provide services in all three cities:

Spokane, Richland and Yakima, Washington.

USMS Eastern District of Washington’s Armed Guard/Transport Services 19 | P a g e

• Key Personnel – if key personnel are not proposed must provide a statement how the required services shall be provided without these key personnel (reference PWS

III.A.).

2) PAST PERFORMANCE – The offeror is required to provide past performance data for similar or relevant Government or Commercial contracts worked within the past five (5) years by submitting at least three (3) different sources to enable the Government to verify the offeror’s Past Performance (See Reference List for Past Performance Attachment # 2).

Offerors shall have three (3) of their previous customers complete the Past Performance Questionnaire (Attachment #3). The assessment of the Offeror’s past performance will be used as a means of evaluating the relative capability of the Offeror.

In investigating an offeror's past performance, the Government will consider the information provided by the offeror, information obtained from other sources, including past and present customers and their employees; other government agencies, including state and local agencies;

consumer protection organizations and better business bureaus; former subcontractors; and others who may have useful information.

In the case of an offeror without a record of relevant past performance or for whom information on past performance is not available or so sparse that no meaningful past performance rating can be reasonably assigned, the offeror may not be evaluated favorably or unfavorably on past performance. Therefore, the offeror shall be determined to have unknown past performance. In the context of acceptability/unacceptability, “unknown” shall be considered an “unknown” past performance.

The assessment of the offeror's past performance will be used as a means of evaluating the relative capability of the offeror. Past performance must reflect that work performed in the past five (5) years was satisfactory or better, as well as indicate a specific rating of satisfactory or better on meeting the performance schedule.

3) PRICE –Each proposal will be evaluated on the basis of price reasonableness. This evaluation may include, but is not limited to a comparison of the proposed price with the prices proposed by other offerors, the Government’s estimate, consideration of current prices being paid for the same or similar services, current market conditions, as well as other relevant measures. This factor shall be addressed in the solicitation's Price Schedule.

AWARD:

The contract will be awarded to the offeror whose quote contains the combination of evaluated factors (i.e.

Technical Capability, Past Performance, and Price) that is most advantageous to the Government.

The Government will perform a trade-off analysis, taking the established factors into account. The trade-off determination entails an assessment by the Government, in which, Technical Capability and Past Performance (when combined) are significantly more important to Price.

All responsible sources that can meet the requirements and provide items as listed above may respond to this solicitation by submitting a price quote and technical proposal. To be eligible for award under this

USMS Eastern District of Washington’s Armed Guard/Transport Services 20 | P a g e solicitation, the offeror must be listed with active status on the System for Award Management website -www.SAM.Gov.
