The estimated volume is 4 ambulance calls per week (16 per month), though the contractor must develop a contingency plan to accommodate unforeseen catastrophic incidents. The contractor must maintain current Louisiana licensure for BLS and ALS ambulance services, comply with DHH 105 CMR 170.305(c) staffing requirements, and ensure all EMTs, paramedics, and ACLS staff are properly trained, licensed, and insured. Monthly reporting to the Contracting Officer's Representative is required documenting total calls requested, calls responded to, and calls deferred to local EMS providers.

Payment is structured as a fixed/capitated price per transport with mileage limited to one-way distance up to 20 miles; trips beyond 20 miles are billed at a rate per mile that includes base rate plus distance surcharge. Cancelled trips for which the vehicle has been dispatched or trips not completed due to factors beyond the contractor's control receive 100% of the base rate. All ferry, bridge, tunnel, and toll charges are included in pricing with no separate billing permitted. The contractor must submit consolidated monthly bills via electronic funds transfer and provide individual run sheets and itemized bills upon request. The contractor must be registered in SAM.gov, provide current insurance certificates at solicitation and each option year, and acknowledge that contract validity depends on the vendor remaining the authorized emergency service provider for Grant Parish.
