(15BCTS25R00000009 Community Treatment Services in Philadelphia, PA)
The “performance questionnaire” looks new to us and so we have a few questions.
1. Are we to ask any agencies we work with in Philly to fill this out?
Absolutely! Or you can fill it out for them. It’s your choice.
2. What happens they don’t complete it? Can we submit an audit or monitoring report instead or should we fill it out?
You are more than free to complete and submit an audit or monitoring report.
3. I see that its’ due as a separate attachment via email. Should we have more than one, should we merge into one file to attach or attach separately?
Electronic copies of proposals shall be submitted via email to Kacannon@bop.gov and r1carroll@bop.gov in Adobe Acrobat (PDF) format. Email attachments (PDFs only) cannot total more than 14 MB per email.
Please compress PDFs when possible, to reduce the file size. We understand you will likely need to send multiple emails. Please confirm with the Contracting Officer that all emails and attachments were received.
Offers will only be accepted via email. Please include SEPARATE FILE ATTACHMENTS for the following:
Completed SF-1449 Completed 2.1, Continuation of SF-1449 Completed Business Proposal Information Packet
Completed Technical Proposal Information Packet Completed Past Performance Questionnaire Proof of completed SAM registration (must be active by time of proposal submission) A copy of your current Professional Liability Insurance Completed Proposal Attachment Form
4. What should we do if we don’t have any other customers in Philly to fill it out? Just have a few references complete.
Unfortunately, this is a determination your agency must consider. Please address each item in the requirement as it’s own.
5. The form required for the proposal called “past performance questionnaire” is new to me. Is this required to be filled out by any agency we provide specialized services for in Philadelphia? Do we/should we include BOP? Or should I have a couple of our contract contacts filled out from other states?
I hope this correspondence finds you well and in good spirits. Thank you for your continued interest in our recent requirement of 15BCTS25R00000009 Community Treatment Services in Philadelphia, PA.
Please allow me to clarify. If I understand you correctly, you’d like to know if the Past Performance Questionnaire is required to be filled out by any agency, we provide specialized services for in Philadelphia? Do we/should we include BOP?
The Past Performance Questionnaire can be filled out by any agency or by you and can/may include existing /expired BOP contracts.
