PART I – THE SCHEDULE
SECTION B – SUPPLIES OR SERVICE AND PRICE / COST
Cell-Based Skin Substitutes for Definitive Care of Burn Injuries
B.1. BRIEF DESCRIPTION OF SUPPLIES OR SERVICES
The Biomedical Advanced Research and Development Authority (BARDA) has a need to effectively treat and ameliorate the effects of burn injuries resulting from the detonation of a nuclear device. Treatment and care for burn injuries is labor intensive and influenced by the unique properties of each case with complicating factors including patient age and health as well as burn area and depth. The challenges of providing definitive burn care are heightened when delivering treatment after a mass casualty event in a resource strained environment. The number of patients generated by a mass casualty event will create bottlenecks in the ability to provide quality treatment and care for victims. In an effort to address the additional complications commensurate with a mass casualty event, BARDA has worked with the burn care community to identify those key procedures that can be improved to mitigate the consequences of an event generating burn victims. The two major areas where enhanced treatment options could provide a meaningful difference are in the initial care and triage of patients and secondly, in the long term treatment and recovery steps of burn care. The focus of this solicitation is to address the long term treatment and recovery steps of burn care.
Cell-based skin substitutes requiring a single surgical application (in lieu of autografting) would increase treatment capacity to provide definitive wound closure to burn injuries. The use of skin substitutes particularly during public health emergencies could potentially reduce the time, labor, and resource burdens associated with standard of care. This would free up resources to enable treatment for more patients. Highly desirable skin substitutes are those which are fully resorbable, facilitate definitive wound closure of partial thickness and even full thickness burn wounds, and require no additional surgical procedures after placement on the excised burn wound.
Under this Request for Proposal (RFP), BARDA is seeking to fund late-stage development and procurement of cell-based skin substitutes that can enhance the capacity to provide definitive care for thermal burn injuries. A key potentially desirable attribute of cell-based skin substitutes may be to reduce or eliminate the need for autografting and thereby increase the efficiency of burn care by promoting faster burn wound healing and recovery. Autograft-sparing products will not be considered as a possible solution under this category of products. The late stage development and procurement of autograft-sparing products is the subject of RFP# 15-100-SOL- 00019. All candidate products shall at a minimum be in active clinical Phase II study or pilot study for devices with data available to support the intended use as a cell-based skin substitute for treatment of deep partial and /or full thickness skin burn injuries. Products are to be procured in sufficient quantities at the discretion of the USG to enable an effective emergency response under Emergency Use Authorization or upon the Food and Drug Administration’s (FDA) approval.
B.2. PRICES / COSTS
The final contract will contain the price/cost provisions agreed upon by the Government and the Contractor. It is anticipated that the final contract will be a five year (5) contract which contains a base period of five years (60 months) and three (3) potential options for additional quantities to be potentially exercised within the base period during the life of the contract. Below are the anticipated Contract Line Item Numbers (CLINs). The final contract will contain CLINs agreed upon by the Government and the Contractor.

SECTION C – DESCRIPTION/SPECIFICATIONS/WORKSTATEMENT
C.1. STATEMENT OF OBJECTIVES
1. Background and Purpose
BARDA has a need to effectively treat and ameliorate the effects of burn injuries resulting from detonation of a nuclear device. In an effort to address the additional complications commensurate with a mass casualty event BARDA has worked with the burn care community to identify those key procedures that can be improved to mitigate the consequences of an event generating burn victims. The two major areas where enhanced treatment options could provide a meaningful difference are in the initial care and triage of patients and secondly, in the long term treatment and recovery steps of burn care. The focus of this RFP is to address the long term treatment and recovery steps of burn care.
Under this RFP, BARDA plans to support late stage development and acquire medical countermeasures to address the recovery and healing of burn victims. These treatments will enhance the capacity of burn care specialists to rapidly and effectively treat thermal burn skin injuries. This Statement of Objective (SOO) focuses on allogenic skin substitutes.
For the purposes of this RFP, the following definition of skin substitute by the American Burn Association’s publication “Surgical Management of the Burn Wound and Use of Skin Substitutes,” 2009, has been used. Accordingly, a "skin substitute" is defined as, “a biomaterial, engineered tissue or combination of materials and cells or tissues that can be substituted for skin autograft or allograft in a clinical procedure".
Discussions within the professional burn community and market research data indicates that synthetic allogenic cell based skin substitutes can provide an advantage in effective burn care by promoting healing and recovery of severe burn injuries. Cell based skin substitutes have the potential to reduce the need and/or serve as an alternative to autografting, a painful and highly specialized procedure which involves removal of skin from an uninjured area of the patient’s body to replace the skin damaged by burn. The burnt area of the skin must be removed carefully via a surgical procedure followed by surgical grafting of the healthy skin removed from an uninjured location. The autografting procedure is labor-intensive, requires training, is time consuming and invokes additional risk to the patient due to the operative procedures at multiple sites. In mass casualty scenarios involving significantly high numbers of burn patients, the burn care medical community could not deliver adequate attention and care using current medical practices.
2. Scope:
The USG will support late stage development and procurement of cell-based skin substitutes. A key potentially desirable attribute of cell-based skin substitutes is to reduce or eliminate the need for autografting and thereby increase the efficiency of burn care by promoting faster burn wound healing and recovery. These products are expected to enhance the ability of the USG to respond to a sudden surge in burn care needs after a mass casualty event and will improve the current standard of definitive burn care in the long term.
In order to meet this scope, the objectives are outlined in the following six sections that the Contractor shall address in their proposal:
Section 3: Late stage Product Development Objectives (Licensure, approval, or clearance of product through FDA)
Section 4: Procurement, Storage, or Delivery of Product Objectives
Section 5: Program Management Objectives
Section 6: [Contract Option] Post-Marketing Commitments and/or Requirements
Section 7: [Contract Option] Pediatric Studies
Section 8: [Contract Option] Additional Procurements
Proposals from Contractors would be evaluated in the ability to meet these objectives as described below.
3. Late stage Product Development Objectives
Independently, and not as an agent of the USG, the Contractor shall furnish all the necessary services, qualified personnel, materials, supplies, equipment, facilities, transportation and travel not otherwise provided by the USG as required to fulfill the programmatic objectives.
3.1. General Product Development Objectives
3.1.1. The product shall at a minimum be in active clinical Phase II study or pilot study for devices with data available to support the intended use as a cell-based skin substitute for treatment of deep partial and /or full thickness skin burn injuries. It is possible the FDA may request separate clinical trials for deep partial and full thickness indication. The Contractor shall present sufficient non-clinical and clinical study data to support the use of the product per its intended indication under discussion with the FDA.
3.1.2. The product shall have the potential for use in both adults and special populations (e.g.
pediatrics, immunocompromised, elderly). However, it would be acceptable for the Contractor with product under development to have a plan for addressing the needs for special populations.
3.1.3. The Contractor must define how the proposed product will enhance the readiness of the medical community to treat burn patients in mass casualty event. Enhanced readiness can be evaluated by the ability of the product to address the potential desirable attributes in three areas listed below:
- Decrease resource burden and increase throughput
- Improve patient outcome
- Product sustainability
It is anticipated that a product may not have the ability to meet all desirable attributes. The Contractor is encouraged to provide a clear, candid and justifiable response especially when direct evidentiary data is not available.

3.1.3.3. Product Sustainability
3.1.3.3.1. Integration of product into current procedures and compatible with current clinical skills.
3.1.3.3.2. Potential to reduce the overall cost of burn care for the patient.
3.1.3.3.3. Other potential applications for the product in commercial marketplace and its development status.
3.1.4. As part of the clinical evaluation plan for the product, the Contractor shall conduct a pharmoco-economic analysis of the treatment to assess the impact on the cost of care compared to the alternative or standard of care in an effort to demonstrate the value proposition in burn care.
3.1.5. The Contractor must demonstrate applicability of their product for other indications in the commercial market sector and the status.
3.1.6. The Contractor shall provide a draft Target Product Profile (TPP) which will be further developed through discussion and negotiation with BARDA and / or FDA. Guidance to the industry is available at FDA TPP Guidance to industry:
http://www.fda.gov/downloads/drugs/guidancecomplianceregulatoryinformation/guidance s/ucm080593.pdf
3.2. Regulatory, Non-Clinical, and Clinical Objectives
3.3.
3.3.1. The Contractor shall submit a Quality Management System (QMS) plan.
3.3.2. The Contractor shall provide a clear and comprehensive regulatory master plan for obtaining product licensure. The plan must include risk evaluation and address outstanding items identified by FDA (toxicology, non-clinical and clinical studies, and manufacturing activities). The appropriate milestones for regulatory activities must be included the overall Gantt chart.
3.3.3. The Contractor shall inform BARDA of all communications with the FDA for this specific product.
3.3.4. The Contractor will be responsible for generating the data to support USG filing of a Pre-
Emergency Use Authorization (Pre-EUA) package for use of the product during a declared emergency, declared potential emergency or identification of material threat under an Emergency Use Authorization (EUA). For information concerning EUA, please consult http://www.fda.gov/RegulatoryInformation/Guidances/ucm125127 and http://www.fda.gov/EmergencyPreparedness/Counterterrorism/MedicalCountermeasures/M CMLegalRegulatoryandPolicyFramework/ucm182568.htm http://www.fda.gov/downloads/drugs/guidancecomplianceregulatoryinformation/guidances/ucm080593.pdf http://www.fda.gov/downloads/drugs/guidancecomplianceregulatoryinformation/guidances/ucm080593.pdf http://www.fda.gov/EmergencyPreparedness/Counterterrorism/MedicalCountermeasures/MCMLegalRegulatoryandPolicyFramework/ucm182568.htm http://www.fda.gov/EmergencyPreparedness/Counterterrorism/MedicalCountermeasures/MCMLegalRegulatoryandPolicyFramework/ucm182568.htm
The information and data needed to support the Pre-EUA package will be determined by FDA. The Contractors shall note that the submission to support potential use under an emergency will be submitted by BARDA or the CDC/SNS, and the Contractor(s) shall be obligated to supply BARDA or the CDC/SNS with the data needed to support such a submission, including a right of reference to the Contractor's Investigational New Drug (IND) or other application that contains the supporting data.
3.3.5. The Contractor shall address any FDA comments on pre-EUA package as applicable.
