RFP: 14-233-SOL-00119
AMENDMENT OF SOLICITATION NO.: RFP: 14-233-SOL-00119
AMENDMENT NO.: 03
EFFECTIVE DATE: August 8, 2014
ISSUED BY:
Program Support Center Office of the Assistant Secretary for Administration U.S. Department of Health and Human Services 12501 Ardennes Avenue Suite 400 Rockville, MD 20857
Point of Contact: Shanelle Jackson, Contract Specialist
NAME AND ADDRESS OF RESPONDENTS: To All Responders
The above numbered Solicitation is amended as set forth below.
DESCRIPTION OF AMENDMENT
PURPOSE: To clarify the Solicitation requirements by amending Section L.2; and responding to questions submitted under the Solicitation. Unless specifically noted, all specifications remain unchanged.
Solicitation Amendment:
1. Section L.2 Inquiries/Submission paragraph 4 is revised as follows:
The proposal must be submitted no later than August 28, 2014 at 2:00pm Eastern Time.
2. Questions:
Question 1: For Section B.4. Labor Rates table, are we allowed to put ranges for each class or should we just put a maximum rate for each category?
Answer 1: The Offeror shall incorporate the ceiling rates proposed in Section B.
Question 2: For Section B.4. Labor Rates table, should the rate for each classification have to be presented in a stepwise fashion? For example, is it ok to have a higher labor rate for Class 6 than Class 4?
Answer 2: Rates for each classification should are left to the Offerors discretion. The Government anticipates that these rates will correspond with the Labor Class Definitions provided in Section C.4.
Question 3: For Pricing Scenario attachment, labor rates, should we put maximum labor rate or can we put a mid-point/average for each classification?
RFP: 14-233-SOL-00119
Answer 3: See answer #1 above.
Question 4: For Pricing Scenario, are we allowed to change the number of hours or the amount proposed for Subcontracts?
Answer 4: No, the number of hours and subcontract amounts are being used as a sample only and should remain the same for all offerors.
Question 5: It is clear that the electronic versions of the full proposal are due at noon Eastern Time on August 28, 2014. However, I would appreciate clarification for the addition of the requirement for 7 hard copies of the technical proposal volume only (not including the CV/Resume appendix). Given that we will be shipping these hard copies from Colorado it would be very helpful to know if the 7 hard copies must also be delivered by noon Eastern Time on August 28, 2104 or if they can arrive after the noon due date as long as the shipping company has logged the package into their tracking system before noon on August 28th.
Answer 5: All items must be received at the Government office designated in the solicitation by the time specified in the solicitation. See amendment #1 above.
Question 6: The Armstrong Institute wants to make sure they are understanding who exactly can provide references for the Past Performance section of the ACTION III RFP. Can you please clarify what groups are able to provide past performance references? For example, can the following provide references:
1. Those that have funded us
2. Those we’ve collaborated on projects with
3. Those who we’ve acted as a subcontractor for
4. Those that evaluated us
Are there additional groups? Also, does it have to be based on our performance on only contracts, or can grants be included? If you can confirm that we understand this section correctly, we would greatly appreciate it.
Answer 6: References should be provided from organizations which the offeror has done work.
