About this file

This document is a Past Performance Questionnaire (PPQ) from the U.S. Department of the Interior, National Park Service, used to collect feedback on a contractor's previous project performance. The form provides a structured method for evaluating a company's past work, with sections for project details and a comprehensive rating system covering relationship management, corporate integrity, quality control, and schedule performance. Evaluators can rate performance levels from Outstanding to Unacceptable across multiple categories and are asked to provide additional context through comments and specific questions about customer satisfaction, potential performance issues, and willingness to re-engage the contractor.
