About this file
This is a Lowest Price Technically Acceptable (LPTA) submission template for construction work under a Request for Quotation (RFQ). The template is designed for quoters to demonstrate technical acceptability and submit pricing for directional drilling and underground conduit installation projects. To receive a "PASS" rating and remain eligible for award, quoters must complete all required entries; any omission results in a "FAIL" rating and disqualification. Award will be made on an LPTA basis with no tradeoffs, evaluated according to criteria established in the solicitation.
The template requires quoters to provide: (1) company information including legal name, UEI, CAGE code, and primary point of contact; (2) evidence of relevant experience through two completed projects within the last five years involving directional drilling (HDD) and/or underground conduit installation, including customer name, location, project scope, timing, contract number, and reference contact; (3) identification of key subcontractors and their trades if specialized work will be subcontracted; (4) a SOW/Specs compliance checklist confirming compliance with five specific requirements: installing approximately 210 linear feet of 2.5-inch HDPE electrical conduit via directional drilling, installing four 4/0 AWG and one 4 AWG copper conductors, installing a 2-inch HDPE telecommunications conduit with pull tape and tracer wire, using only water-based drilling fluids with proper containment and offsite disposal, and using existing basement building penetrations with Contracting Officer approval for any modifications; and (5) signature and certification by an authorized representative. Each compliance item requires a binary Comply/Does Not Comply checkbox with optional notes explaining any inability to meet requirements.

Does Not Comply
Notes (Why compliance can’t be achieved)
Requirement 1 (Electrical conduit)
Install approximately 210 LF of 2.5” HDPE SDR 11 electrical conduit using directional drilling; use sweeps/HDPE elbows at terminations; run must be one continuous pipe from transformer to building (no fused segments); install tracer wire; install at least 2 feet below grade or minimum depth required by NEC/Greeneville Energy Authority/local codes, whichever is greatest.
Click or tap here to enter text.
Requirement 2 (Electrical conductors)
Install four (4) 4/0 AWG conductors and one (1) 4 AWG ground; THWN-2 copper; 600V rating; 90°C wet/dry;
conductors must be one continuous run from electrical box to main disconnect (no splices).
Requirement 3 (Telecommunications conduit)
Install an additional 2” SDR 11 HDPE telecom conduit suitable for fiber optic cable using directional drilling; run must be one continuous pipe from location adjacent to transformer to building (no fused segments); install with 1500 lb pull tape and a tracer wire; install a new pull box adjacent to the transformer at the starting point of the telecom conduit run to the basement; use HDPE sweeps at terminations.
Requirement 4 (Directional drilling fluids / containment / disposal)
Only water-based drilling fluids may be used; prohibited components include diesel fuel, oil-based additives, petroleum-based lubricants, and any fluid/additive classified as hazardous; contractor must contain all drilling fluids/spoils, prevent discharge (including inadvertent returns), clean up daily and at completion, restore disturbed areas, and dispose of all drilling fluids/spoils at an approved offsite facility (no onsite burial/discharge).
Requirement 5 (Basement penetration / historic fabric protection)
Contractor shall use the existing building penetration in the basement for electricity (existing penetration is under the basement slab); if reuse is not feasible, contractor must submit to the CO the proposed new penetration location and the reasons existing cannot be reused, and obtain CO approval before performing any work; contractor must also provide a submittal detailing the proposed location of the telecom penetration.
5) Quoter Certification (Required)
