Re: Limited Asbestos Survey WT Job No. 2581JE087 1109 North Eighth Avenue Holbrook, Arizona
Western Technologies Inc. (WT) presents this report of the limited asbestos survey for eight residential buildings located at 1109 North Eighth Avenue, Holbrook, Arizona (the “Site”). WT was authorized by Stan Belinte (the “Client”) to perform these services according to WT’s Agreement for Professional Services (WT Contract No. 2581PE082R1, dated March 31, 2021). This report was prepared for the Client and it may not be utilized or relied upon by any other person or entity without the prior written permission of WT. This report concludes our services on this portion of this project.
PURPOSE
The purpose of this project was to report the location, condition, and quantity of asbestos containing materials (ACMs) identified on the Site.
SCOPE OF SERVICES
The scope of services for this project was based on the sampling guidelines of the Asbestos Hazard Emergency Response Act (AHERA) and the National Emission Standards for Hazardous Air Pollutants (NESHAP) as established by the United States Environmental Protection Agency (EPA) and consisted of the following tasks:
A survey of the building by an EPA-accredited asbestos building inspector to assess the location, condition, friability, and quantity of suspect ACMs as requested by the Client.
Petrified Forest National Park April 27, 2021 WT Job No. 2581JE087 Page 2
The collection of bulk samples of suspect materials that according to the Client will be disturbed during the proposed rehab activities. Sampling was limited to exterior stucco and associated wall materials.
The microscopic analysis of the collected samples by a laboratory accredited under the
National Voluntary Laboratory Accreditation Program (NVLAP), using Polarized Light Microscopy (PLM).
The preparation of this report, describing the field survey and categorizing the identified materials based on the laboratory analyses.
FIELD SURVEY
The Employee Housing Rehab Project in Holbrook, Arizona was sampled on April 2, 2021 by Greg Kendall, who is an EPA-accredited building inspector. A copy of the inspector’s accreditation is presented in Appendix A. The inspector observed the building materials to identify and describe suspect ACMs, record their locations, and evaluate the condition and friability of each material.
Sampling was limited to the exterior walls of buildings, fences, and pillars.
SITE DESCRIPTION
Eight single-story residential buildings that are constructed with a plaster/stucco, brick and mortar exterior and shingled roofs.
LABORATORY ANALYSIS
A total of 27 samples were collected from the Site. In some instances, the bulk samples consisted of more than one layer, and the laboratory analyzed all discrete layers in the samples. WT submitted the collected samples to Steve Moody Micro Services, LLC (SMMS) in Farmers Branch, Texas who is accredited through the NVLAP (102056-0). SMMS analyzed the samples according to EPA Method 600/R-93/116 (the New Method) PLM with dispersion staining and point counted.
The samples were analyzed on a 5-day laboratory schedule. The data provided by the laboratory was used to identify those suspect materials containing greater than 1% asbestos. A summary table of the materials sampled is presented in Appendix B. The laboratory report and chain-of-custody record are presented in Appendix C and a site plan depicting the room designations is presented in Appendix D.

RESULTS
Asbestos-Containing Materials (ACM):
Building Location Material Estimated Quantity*
NESHAP OSHA
Exterior Walls, Fences, Pillars Stucco ~29,284 ft2 (spread over 8 buildings)
CAT II Class II
Notes: *Contractors should not use footages for biding purposes. All footages should be verified by contractor prior to demolition.
Cat II denotes Category II
OCCUPATIONAL SAFETY AND HEALTH ADMINISTRATION (OSHA) CLASSIFICATION AND NESHAP
CATEGORY
The following paragraphs provide classifications and categories used to describe the regulatory requirements for the disturbance of ACMs. The OSHA classifications provide details for the personal protective equipment and engineering controls needed for abatement of these materials. The NESHAP categories prescribe waste management, emission control, and disposal requirements. The selection of removal methods, including the use of manual or mechanical removal methods, can affect the NESHAP category of materials.
Stucco OSHA classifies the removal of asbestos-containing stucco as Class II work. The stucco is categorized by NESHAP as a category II, non-friable asbestos-containing material.
RECOMMENDATIONS
Based on the results of this survey, WT states the following recommendations:
A NESHAP notification is required 10 working days prior to general demolition activities that will affect a load bearing wall.
A licensed abatement contractor should be retained to perform the abatement of the identified and Category II, non-friable materials.
Document all correspondence from the abatement contractor and the testing laboratory and retain a permanent record.
If materials other than those sampled as part of this survey are encountered during demolition, they should be assumed positive until they are sampled and tested.
WT Job No. 2581JE087 Page 4
LIMITATIONS
Conditions can exist within structures and below the ground surface that are not apparent visually or disclosed by sampling data. This study is limited to the conditions expressly disclosed in this report, and it does not represent the assessment or absence of any other conditions on or affecting the Site. WT’s findings are based on the assumption that the sampling locations, and the resulting data, are representative of assessed conditions. WT’s interpretation, discussion and opinions of the results obtained from the referenced methods, observed conditions, and tested samples are applicable only to the specifically tested locations at the times stated herein.
The regulatory standards referenced in this report are based on our knowledge of applicable regulatory standards in effect at the time the work was performed. WT cannot anticipate potential future changes to regulatory standards by appropriate governmental agencies.
WT has performed our services in accordance with our contract with our Client, utilizing the ordinary degree of skill and care practiced by other firms providing similar services in the locality of the site. No other warranty or representation, either expressed or implied, is made.
