About this file
This statement of work outlines the plugging and abandonment of eleven orphan oil and gas wells located in Grand County, Utah. The Bureau of Land Management seeks to isolate usable water zones, protect mineral deposits, and properly plug and abandon the wells in accordance with industry standards and all applicable regulations. Work includes setting cement plugs to isolate formations including the Castlegate, Mancos, and Dakota, perforating and squeezing casing, circulating cement to surface, installing dry hole plates, and reclaiming well sites. Contractors must provide plugging equipment, rigs, wireline units, and cementing equipment capable of working to depths up to 7,000 feet and pumping specified cement volumes.

I . SUMMARY OF WORK:
Eleven wells in Grand County, previously operated by Northstar Gas Company Inc, have been placed into an orphan status with no idenfiable responsible party. In accordance with the requirement of 43 CFR 3162.3-4 (a) (b) and per Secon XII of the Bureau of Land Management Onshore Oil and Gas Order No.
1, III., XII., A., B., these wells must be plugged and abandoned (P&A), the locaons cleaned up, and the surface reclaimed.
This report contains general lease informaon for each well, the geological formaons, the locaon of the wells, wellbore design, a procedure to P&A each well, and reclamaon plans for the wells.

Contractor shall perform work in accordance with industry standards and all local, state, and federal regulaons. In general, orphan wells shall be plugged following the standards established in Onshore Oil and Gas Order No. 2 [54 FR 39528, Sept. 27, 1989], Secon G. Drilling Abandonment Requirements.
Hydrogen Sulfide Gas (H2S) is not known to be present in these wells. Should H2S be detected in concentraons over 100 ppm in the gas stream, operaons will be shut down unl a plan to safely proceed is developed and implemented.
I I . BACKGROUND:
These eleven wells were drilled and completed by Belnorth Petroleum Corp between 1960 and 1981.
The wells changed hands mulple mes through the 1980s and 1990s before being taken over by Northstar Gas Company Inc in 1994. The wells connued producing unl March 2013 when the first well
| 5 was shut in. All eleven wells were shut in by January 2016. Northstar Gas Company has been unreachable for several years and the wells have been classified as orphaned. The BLM has both surface and mineral responsibilies for all involved wells. Because the involved lessees/operators failed to properly plug and abandon the wells, the BLM is now responsible for proper plugging of the wells.
1. GEOLOGY:
Onshore Oil and Gas Order No. 2 Secon III., G. states that “all formaons bearing usable-quality water, oil, gas, or geothermal resources, and/or a prospecvely valuable deposit of minerals shall be protected.”
Utah Geological Survey, Study 144 Plate 4, Depth to the BMSW, Uinta Basin, Utah published by Utah Geological Survey in 2012 lists the depth to the base of moderately saline waters in northern Grand County in the area of these orphaned wells at approximately 3000-5000 feet below the surface. Aer evaluaon it was determined that the BMSW is below a usable-quality depth and thus will not be isolated with a dedicated plug at that depth. Plugs above that depth will isolate it. Table 2 below lists the formaons that were penetrated by eleven orphaned wells. These formaons will be isolated.
Table 2 – Formaon top depths. Colors indicate the formaons in which water or oil/gas are encountered, and perforated in each wellbore:
NR= Formaon top Not Recorded in drilling reports with UDOGM; NDE= well Not Deep Enough
Water recorded Gas/oil recorded Perforaons
2. WELL LOCATION:
The orphaned wells, shown in Figure 1 below, are located in Grand County, Utah approximately 60 miles west of Grand Juncon, Colorado. The orphaned wells can be found at the latudes and longitudes listed in Table 4 below. A locaon map and direcons to each site are atached.

See “current wellbore schemac” for each well, atached below.
I I I . PLUGGING AND ABANDONMENT PLAN:
The contractor shall have a copy of the contract on locaon at all mes, while performing operaons.
GENERAL
A. Descripon: The plan submited by the Contractor shall provide for the plugging and abandonment of an orphaned oil and gas well situated on land in a semi-arid environment.
B. Submitals: All submitals shall be signed and dated by the Contractor and idenfied by the project number. Submitals requiring BLM approval will be returned to the Contractor within ten working days by the Contracng Officer’s Representave (COR). Submitals by the contractor to the BLM shall include:
1. Records: A daily tour sheet (driller's log) shall be kept on the work site. This sheet must keep a current descripon of all acvies conducted during each 24-hour period. All service company's reports must also be kept at the work site. All records shall be available for inspecon during the job, and shall be submited to the COR at the conclusion of the job or upon request.
2. Operaonal Plan: The Contractor shall provide a work schedule with any updated changes daily. The BLM Project Inspector (PI) will be on the work site during the plugging operaon to ensure that the contract is followed and that needed approvals for changes can be expedited.

MINIMUM REQUIREMENTS REQUIRED BY REGULATION: Onshore Order No.6 – H2S Operaons
1. General
a. A specific H2S operaons plan for plugging operaon will not be required for approval prior to starng work, personal H2S monitors will be worn on locaon.
b. If H2S is encountered in excess of 100 ppm in the wellbore, the following measures shall be taken:
i. The Contractor shall immediately ensure control of the well, suspend plugging operaons and obtain materials and safety equipment necessary to bring the operaons into compliance with of applicable provisions of Onshore Order No. 6.
ii. The Contractor shall nofy the COR of the event and the migang steps that have or are being taken as soon as possible.
iii. It is the Contractor’s responsibility to ensure that the safety plan is approved by the CO and the contract amended, prior to the resumpon of operaons.
