AMENDMENT #1: RFP No. 140L0621R0036 Utah Wells 2021 Plugging & Abandonment

ATTACHMENT 6—SITE INSPECTION TOUR/PRE-BID CONFERENCE

RECORD

Pre-site visit conducted on August 24, 2021 by Ryan Angus, BLM Petroleum Engineer and Contracting

SITE VISIT NOTES:

Some of the wells in the requirement were visited and others discussed. Notes and photographs of the well requirements discussed are provided herein. See also Attachment 1 – Amendment 1- Statement of

Work, revision posted with this Amendment.

Wells shall be plugged in the order listed in the contract. It is anticipated that most of the option wells will be elected as BLM would like to see the Book Cliffs wells are plugged. Water on location after plugging a well shall be hauled off to disposal if it has been used in the well for anything.

Table Top 1

No site visit was planned from this location. Contractors are welcome to visit the location if they would like to. Ryan Angus visited the location on August 23, 2021 and made the following notes about this well:

1. The location is approximately 520 ft X 220 ft. There is pile of road base on location that is approximately 450 cu yards of road base/gravel, and 6 inches of road base/gravel (2120 cu yards) is to be ripped up and removed, then hauled to the gravel pit located 2.5 miles down the road. This makes a total of 2570 cu yards of gravel will be moved from location to the pit. After gravel is removed, the location will not be re-contoured, but will be ripped and seeded with a seed mix provided by the Forest

Service. Moving gravel should be bid as a $/cu yard.

2. The access road is to be recontoured from the location to the first bridge, 0.7 miles from location. If possible, 6 inches of gravel shall be picked up and moved to the same location as the previous gravel.

This equates to approximately 550 cu yards of gravel to be removed from the road prior to recontouring. Moving gravel should be bid as a $/cu yard.

3. The well head is not standard, see picture and casing is not to be cut and recovered. All plugs above the casing cut point will be pumped inside/outside with perforations at the bottom of the plug and a

CICR in the middle.

4. There is a water well on location which consists of 8” pipe about 300’ deep. If the contract desires, they can acquire a water right from the State of Utah and used this fresh water on location. There is no information on production flow rates and thus the well may not be able to supply the necessary volumes needed to plug the Table Top well. Bids should not include using this water to plug the wells. If it works out to use the water, adjustments will be made after the plugging. After plugging the table top well, this well will be filled up with cement pumped from surface by the contractor.

Price wells

These wells are to be plugged according to the RFP. Contractor should plan for a work string to be delivered to location since records are not very good and there isn’t a lot of possibility that a well will have a tubing string in it that can be used. Instead of rig anchors, “T” Seals or Base Beams are acceptable. (This applies to all wells with minimal reclamation work to be performed.)

MOAB wells

Megadon Federal 1-15

1. Fill in the road (temporary) with a 4 ft culvert to gain access to location. Pull the culvert and remove the gates/panels when work is complete.

2. Tanks have some fluid and solids in them. Plan to clean tanks before moving them.

3. Push the East edge of the location down the hill toward the pit at the bottom. Just enough to make the slope more gradual and less noticeable. Pull the berm surrounding the pit below the pad in to fill the pit a little and to eliminate the berm.

4. Clean up the trash and extra materials on location and on the old plugged well adjacent to the location. Also, fill in the rat hole on that location.

5. Plan to cut tubing at 8230 ft and proceed with plugging procedure.

TXO 9-1

1. This well can be plugged riggless. Cap CIBP at 1620’ with 35’ cement with a bailer. Pump plug 2 without a CICR and tag TOC with wireline. Pump plug 3 as detailed.

2. No reclamation work is needed. Just pull the 2” pipe out of the ground and haul it off. Cut off the anchors below the surface and clean up the trash/pipe around the well head.

Alice 29-3

1. Have a blade clean up the access road with a single pass to cover the washes.

2. Close the two small pits by pushing the spoils back into the pits. No other dirt work is necessary.

Book Cliffs 1

1. A little road work is needed to allow the rig to access the location. Also, some brush and vegetation will need to be removed from location to grant access to the well head.

2. No reclamation is needed. Just fill in the two small pits and haul off the separator, tank, and other trash/stuff.

3. Change to plug 1 of procedure: Set a CIBP @5136’ and cap with 50’ cement.

Book Cliffs 3

1. The road is 3,219 ft long and is completely overgrown and washed out. It will have to be reconstructed in order to gain access. The existing gas pipeline will have to be removed prior to starting dirt work on the road. Road construction will include a culvert over the stream and fill in the wash right

140L0621R0036 Attachment 6 Site Inspection/Pre-Bid Tour Notes Page 12 of 13 off the main road. This culvert will be removed after work is completed. There is not a lot of available soils for filing in the washes, but there should be sufficient as long as the road is constructed as temporary and just to allow for access of plugging equipment to plug the well. There are a couple of boulders that will probably need to be blasted and broken up. The remains can be used to fill in some of the washes in the road path. Dirt work is extensive and if fill dirt is needed, price per yard of fill dirt is how it needs to be bid. All vegetation shall be pushed to the side of the road and re-spread upon exit from the well pad. This work is extensive and will require a very large amount of work to be done to get to the well pad. At the well pad, the road will access the pad on the east side of the location behind the tank.

2. The location will need to be cleared of vegetation in order to access the well head. This vegetation shall be stored on location and then respread over the location upon exit from the well pad. No recontouring is needed for reclamation.

3. All facilities shall be removed from the location and the buried pipes shall be cut off and capped 2 ft below grade.
