STATEMENT OF WORK (SOW) – PR 0040429342

SCOPE

1.1 Introduction

This Statement of Work (SOW) describes the necessary activities to support work at the Idaho National Laboratory (INL) to collect and perform new subsurface data collection. The data collection would be done for two boreholes, drilled near the Materials Fuel Complex (see Attachment 1), to depth at or near 750 ft below land surface (BLS). The objectives include: 1) obtaining high-quality seismic velocities (S- and P-wave velocities and Poisson’s ratio) from the top of basalt bedrock to bottom-hole depth; 2) identifying lithology that corresponds to velocities through coring and geophysical logging; and 3) providing thorough documentation of drilling, borehole completion, logging, laboratory testing, methods used, data, uncertainties, and interpretations.

The Contractor activities include technical consultation and direction throughout the installation, inspection, and testing of components.

1.2 Work Included

The Contractor will provide technical personnel as appropriate to provide technical support to the United States Geological Survey (USGS) during the installation of high density liners in boreholes at the INL Site.

The Contractor will:

• Evaluate the USGS-planned installation designs to determine if any additional liner components are required.

• Supply high density liner material for collection of seismic measurements, see Attachment 2.

• Install and remove flexible liner material for the collection of seismic measurements.

• Assist USGS personnel in their assessment of the readiness/suitability of the boreholes for installation of liner systems.

o This assessment will include a review of proposed design.

o This assessment will result in recommendations for either proceeding or making improvements before proceeding.

• Complete a pre-installation log, identifying all necessary components and the order of installation.

• Provide a briefing to project personnel before beginning installation.

o The briefing shall address the liner system installation, operation, and maintenance.

• Inspect and test components, as applicable, before installation.

• Provide technical assistance throughout installation of the systems into the boreholes.

• Provide verification to the USGS that liner components are installed correctly.

1.3 Work Excluded

The scope of work excludes any activity not defined or associated with the activities listed in Sections 1.1 and 1.2. Activities that the Contractor is specifically excluded from are:

• Operating heavy equipment (such as drill rig, fork lift, and boom truck)

• Hoisting and rigging

3 TECHNICAL AND FUNCTIONAL REQUIREMENTS

Specific Contractor work activities to be completed during each installation are detailed in this section.

3.1 Pre-job Briefing

The Contractor shall provide a pre-job briefing to the USGS personnel performing the installations. The pre-job briefing will include an overview of the installation planned for each location and a detailed explanation of the installation process.

3.2 Training

The Contractor shall provide training as requested by the field team leader.

The Contractor shall provide an explanation of the system and components.

3.3 Installation Supervision

The Contractor shall inspect and test each component to verify that the components are in working order.

The Contractor shall be onsite during the installation. During the installation, the Contractor will be responsible for supervising the installation by USGS personnel

PR 40429342 - Page 3 of 7 to ensure that liner systems are installed correctly and to the correct depths specified by the USGS.

4 ENVIRONMENTAL, SAFETY, HEALTH, AND QUALITY ASSURANCE

REQUIREMENTS

4.1 Environmental

As directed by the USGS onsite.

4.2 Safety and Health

The Contractor will be subject to the health and safety requirements of the USGS Site Safety and Job Hazard Analyses Plan and the USGS Job Hazard Analysis No. 16. The Contractor will be escorted to and from the work site by a USGS escort. The Contractor must maintain a line of sight contact with the escort at all times.

4.3 Quality Assurance/Control

The Contractor shall work with USGS to visually inspect each components and ensure the liners hold fluid.

After the installations are complete, the Contractor shall work with USGS to remove the liner material once the measurements have been completed and verified.

4.4 Training

The Contractor will be trained according to the USGS Site Safety and Job Hazard Analyses Plan and complete any additional training requirements identified by the

USGS.

5 RESPONSIBILITIES

Responsibilities of the Contractor include, but are not limited to:

• Complete training prior to arrival onsite.

• Deliver, or arrange for delivery of personnel necessary to complete the activities described in this SOW.

• Comply with all security, safety, and environmental requirements while onsite.

• Ensure that personnel have the appropriate personal protective equipment and other equipment necessary to complete their work.

• Review USGS-planned designs.

• Inspect and test components.

• Inspect all equipment, tools, and materials prior to mobilization and while onsite to ensure suitability for operation.

• Train USGS personnel (as required).

• Provide technical representative(s) for installation oversight.

• Supervise USGS personnel to ensure correct installation.

• Provide the USGS with completed installation logs and functionality test results upon completion of installation activities.

Responsibilities of USGS include:

• Provide personnel to escort the Contractor while on the INL Site.

• The Contractor will be escorted at all times by USGS personnel.

• Supports well design activities.

• Provide training on the USGS Site Safety and Job Hazard Analyses Plan and any other INL-specific training as required by the USGS work control documentation.

• Provide drill rig for the installation (as needed) and installation of liner systems.

6 SCHEDULE

6.1 Field Schedule

The Contractor will be onsite for the two installations on or near the date July 8, 2019. Expect two weeks to complete both installations and set of measurements.

If there is a change in the field schedule USGS will notify the Contractor fourteen days in advance.

If training is necessary for personnel who have not been onsite at the INL, then the Contractor must be onsite prior to the installation for training.

NOTE: Initial orientation is only conducted on Mondays.

6.2 Deliverable Schedule

The Contractor shall provide the following deliverables:

• Notice of compliance with training requirements identified by the USGS

7 SUBMITTALS

None identified.

8 ACCEPTANCE

The Contractor shall complete installation and removal of liner assembly. Contractor will be responsible for bring equipment to the job site for completion of work.
