About this file
This document is a set of Groundwater Sampling Data Sheets for the Tuba City Groundwater Monitoring project (Project Number EN16-009, Task 001) conducted by the Bureau of Indian Affairs. The data sheets cover multiple monitoring wells (MW-15, MW-28, MW-32, MW-216, MW-301, MW-302, MW-303, MW-304, and MW-311) located in Tuba City, Arizona, and were collected on 7 November 2017 by personnel Ralph Waddington and Howard Small.
The sampling data sheets include detailed technical information for each well, such as depth to water, well depth, well diameter (primarily 2-inch), sampling method (bailer), and water quality measurements including pH, dissolved oxygen, specific conductance, temperature, and ReDox levels. Additionally, the document includes a set of questions and answers related to the groundwater monitoring project, addressing issues such as locating wells, well construction details, public communication plans, and confirmation of regulatory closure for the OU-1 LUST (Leaking Underground Storage Tank) site. The project appears focused on monitoring and potentially decommissioning groundwater monitoring wells in the area.

Questions received and the Government’s Response:
1. We understand that MW-216 and MW-311 are not visible at the surface. Survey coordinates could provide for 100% success in finding the two “missing” wells, especially if the well vaults are missing for some reason and not identifiable with a magnetometer. Does a groundwater monitoring report or other investigation report include surveyed well survey coordinates?
No coordinates are available. BIA will continue researching reports for coordinates. These two wells are located inside and close to a perimeter fence. When a well could not be located, field personnel used hand tools (e.g., shovel, broom, rake) to remove soil and expose associated well monuments.
2. Well construction is needed to develop a plan of abandonment and costs. Is there a table or ‘as-builts’ available?
No as-builts are available. Attached are groundwater sampling data sheet with depth to water and well diameter size for help estimate costs.
3. Is there further guidance on the Public Communication Plan requirements?
The objective of this plan is to inform the public about the nature and duration of the work being performed in the OU-1 area. No additional guidance is required beyond basic awareness.
Communication will be kept simple and direct, such as posting flyers in the immediate vicinity of the work zone or distributing them to employees working nearby.
4. A copy of the closure report would be helpful.
See attached “Plugging Record” from a previous demo project.
5. Before abandonment commences, should we sample MW-216 and MW-311 if we find them and they are in operable conditions?
Based on the regulatory review, OU-1 LUST and associated well are deemed eligible for closure.
No additional sampling is required, and the existing monitoring wells can be decommissioned.
