About this file

This document is a Past Performance Questionnaire for a federal contract opportunity with the National Oceanic and Atmospheric Administration (NOAA) to provide Atlassian Software Maintenance and Renewal services.

The questionnaire is to be completed by references providing information on the offeror's past performance. It requests details on the project/contract, including the title, procurement vehicle, pricing type, period of performance, and contract amount. The reference is asked to objectively assess the offeror's performance across multiple criteria such as quality, business relations, management of key personnel, compliance with requirements, and cost management. The reference is also asked several yes/no questions related to the contractor's past performance, such as whether a contract discrepancy report was issued or the contract was terminated. An overall performance rating is requested at the end.

Please identify and correct any information that is not accurate in Section A:

Please objectively assess the requesting firm in each performance element below by assigning an “X” to the most appropriate rating:

Exceptional: Performance meets contractual requirements and exceeds many to the Government’s benefit. The contractual performance of the element or sub-element being evaluated was accomplished with few minor problems for which corrective actions taken by the contractor were highly effective.

Very Good: Performance meets contractual requirements and exceeds some to the Government’s benefit. The contractual performance of the element or sub-element being evaluated was accomplished with some minor problems for which corrective actions taken by the contractor were effective.

Satisfactory: Performance meets contractual requirements. The contractual performance of the element or sub-element contains some minor problems for which corrective actions taken by the contractor appear or were satisfactory.

Marginal: Performance does not meet some contractual requirements. The contractual performance of the element or sub-element being evaluated reflects a serious problem for which the contractor has not yet identified corrective actions. The contractor’s proposed actions appear only marginally effective or were not fully implemented.

Unsatisfactory: Performance does not meet most contractual requirements and recovery is not likely in a timely manner. The contractual performance of the element or sub-element contains a serious problem(s) for which the contractor’s corrective actions appear or were ineffective.

N/A: Unknown risk would be the equivalent to the evaluator providing a “Not Applicable (N/A)” to any of the questions.

*Failure of customers to provide an explanation to "exceptional," "marginal," or "unsatisfactory" ratings may result in a lower rating.
