Certification of Vaccination The purpose of this form is to take steps to prevent the spread of COVID-19, to protect the health and safety of all Federal employees, onsite contractors, visitors to Federal buildings or Federally controlled indoor workspaces, and other individuals interacting with the Federal workforce. If you fail to submit this signed attestation or any required negative COVID-19 test, you may be denied entry to a Federal facility.

My Vaccination Status By checking the box below, I declare that the following statement is true:

I am fully vaccinated.1

1 The Centers for Disease Control and Prevention considers an individual fully vaccinated if they are:

2 weeks after their second dose in a 2-dose series, such as the Pfizer or Moderna vaccines, or

2 weeks after a single-dose vaccine, such as Johnson & Johnson’s Janssen vaccine

If you don’t meet these requirements, regardless of your age, you are not fully vaccinated.

I am not yet fully vaccinated.2

2 Either I have received my first dose of Moderna or Pfizer, and my second appointment is scheduled, or I received my final dose less than two weeks ago.

I have not been vaccinated.3

3 If you are not vaccinated due to medical or religious reasons, please check either “I have not been vaccinated” or “I decline to respond.”

I decline to respond.

I understand that if I decline to respond or am not fully vaccinated, I must comply with the following safety protocols while in a Federal facility:

Wear a mask regardless of the level of community transmission;

Physically distance; and

Provide proof of having received a negative COVID-19 test from within the previous 3 days if I am a visitor or I am an onsite contractor who is not enrolled in an agency’s testing program.

I sign this document under penalty of perjury that the above is true and correct, and that I am the person named below. I understand that a knowing and willful false statement on this form can be punished by fine or imprisonment or both (18 U.S.C. 1001). Checking “I decline to respond” does not constitute a false statement. I understand that if I am a Federal employee or contractor making a false statement on this form could result in additional administrative action

Directions and notice to Federal employees Consistent with guidance from the Centers for Disease Control and Prevention (CDC) and the Safer Federal Workforce Task Force, has established specific safety protocols for fully vaccinated people and not fully vaccinated people, respectively. You may be asked to submit this completed form to your employing agency.

In areas of low or moderate transmission, as defined by CDC, fully vaccinated people generally can safely participate in most activities, indoor or outdoor, without needing to wear a mask or maintain physical distance, and do not need to undertake regular testing—please note that consistent with CDC guidance, agencies may have different protocols for fully vaccinated people in specific work settings, such as healthcare settings. In areas of high or substantial transmission, everyone, including fully vaccinated people, must wear a mask consistent with Federal requirements.

Employees who disclose that they are fully vaccinated will comply with agency guidance for fully vaccinated individuals. The annual preventative maintenance of each of the systems shall include:

1. Run the chamber through its entire operational range to check the operation of all functions after the completion of all preventative maintenance.

2. Check the refrigerant system for leaks and the operation of the safety switches.

Verify the various refrigeration pressure readings, including oil pressures, on both the high and low stage meet the manufacturer’s specifications. Record these values for inclusion in a Preventative Maintenance Report. Recharge the refrigerant for each system based on readings taken during preventative maintenance when necessary.

3. Measure voltage and amperage readings where required. Inspect all electrical connections and tighten if needed. Record all voltage and amperage readings.

4. Clean the air-cooled condensers units.

5. Clean out the steam generator and humidity systems components. Record the heater resistance values on the steam generator heater elements.

6. Inspect all solenoid valves and record the findings.

7. Inspect door seal integrity and record findings.

8. Check the calibration of the chamber’s reference sensors (temperature, humidity, pressure) and adjust the output if necessary. The service technician shall use calibration instruments that are traceable through N.I.S.T. (National Institute of Standards and Technology). The Government may request copies of calibration certificates for the equipment used.

9. Attach a calibration sticker (near the system controller) showing the date of the calibration; the next calibration due date; and the name of the technician who performed the calibration.

Additional Maintenance Required for Specific Chambers:

The table below includes required additional maintenance that shall be performed as part of the annual preventative maintenance for specific chambers.

Chamber Type Chamber Specific Maintenance Items

Silver Tenney Environmental Chamber Check the operation of the desiccant dryer system and clean the air filters

Thermotron Altitude Chamber Clean the air-cooled condensers units Change the vacuum pump oil, check the operation of the vacuum pump, and record the amperage values when running

Check the oil level in the circulating fan bearings and add oil if required

Check the function of the dive valve and adjust if necessary

Bemco Salt Fog Chamber Clean out the air saturator, spray tower, and solution atomizing devices

At the completion of all preventative maintenance, the contractor shall run each chamber through a short pre-programed profile for the equipment’s entire operational range to verify that the stability continues to meet the following parameters:

1. Blue Tenney Environmental Chamber (WITR-7.65, s/n 21964)L: ±1°C & ±5% at steady state conditions

2. Silver Tenney Environmental Chamber (WITR, s/n 27800): ±1°C & ±5% at steady state conditions

3. BTRC Tenney Environmental Chamber (BTRC, s/n 26887-09): ±1°C & ±5% at steady state conditions

4. Thermotron Altitude Chamber (FA64CH3030RAC, s/n 43570):

a. ± 1.1 °C for pressures greater than 100 hPa

b. ± 2.0 °C for pressures less than 100 hPa

c. ± 2.66 hPa (better at constant temperatures and pressure)

After the contractor has completed the final check for all annual preventative maintenance, the contractor shall provide a Preventative Maintenance Report for each piece of equipment serviced. This report shall list the maintenance activities performed, present system parameter measurements, and describe any recommended improvements or repairs.
