QUALITY ASSURANCE SURVEILLANCE PLAN
FOR
OPERATIONS, MAINTENANCE AND FACILITY SUPPORT SERVICES
INTRODUCTION
This Quality Assurance Surveillance Plan (QASP) has been developed to evaluate contractor actions while implementing this Performance Work Statement (PWS). It is designed to provide an effective surveillance method of monitoring contractor performance for each listed objective on the Performance Requirements Summary (PRS) in the maintenance contract.
The QASP provides a systematic method to evaluate the services the Contractor is required to furnish.
This QASP is based on the premise the Government desires to maintain a quality standard in operating, maintaining, and repairing facilities and that a service contract to provide the service is the best means of achieving that objective.
The Contractor, and not the Government, is responsible for management and quality control actions to meet the terms of the Contract. The role of the Government is to perform quality assurance surveillance to ensure contract standards are achieved.
In this Contract the quality control program is the driver for product quality. The Contractor is required to develop a comprehensive program of inspections and monitoring actions. The first major step to ensuring a “self-correcting” contract is to ensure that the quality control program approved at the beginning of the Contract provides the measures needed to lead the Contractor to success.
The Quality Control Program shall be approved by the Government. The purpose of the QASP is to promote a robust quality assurance program throughout the period of performance of the Contract.
Performance
Objective Performance Standard Performance Threshold
Basic Corrective maintenance, preventative maintenance, emergency maintenance and recurring maintenance, repair, and services.
Maintain Quality Standards as stated in PWS Section VIII;
Safety, Security, Fire Protection, Environmental Controls, Conservation of Utilities and Compliance with Laws and Regulations.
Not to exceed one (1) valid complaint of unsatisfactory per quarter of Performance Period.
Obtain/maintain a valid NJ Class A/B Operator Certification
• NJ Class A/B UST Operator: An onsite person (preferably the foreman) shall be required to obtain/maintain a valid NJ Class A/B Operator Certification, and serve as the facility's Class B UST operator, as designated by NOAA's UST Registration with NJDEP.
• Class C Operators. Two on site persons shall be properly trained and serve as the facility's Class C UST operator, as designated by NOAA's UST Registration with NJDEP.
• NJ Class A/B/C UST Operator Training Records: In accordance with NJAC 7:14B-5A.5, the Contractor shall provide to NOAA records that document the training and retraining, if applicable, received for each designated Class A, Class B, and Class C operator.
Certification shall not lapse during Performance Period.
Emergency Services
• The Contractor shall be required to provide qualified trades employees to handle emergency services. When emergencies arise, the Contractor shall assign trade employees best experienced at the site of the emergency.
• Emergency call-ins will guarantee a minimum of two
(2) hours payment to the Contractor, exclusive of travel, at the proper rate at the time of the emergency call. Emergency calls require the Contractor to be "on site" within one (1) hour of notification. This service shall be conducted during any hour of the day or night, which may include both overtime and normal working hours.

SURVEILLANCE: The Government COR and the Contracting Officer as may be necessary will evaluate the services required by the contract to ensure compliance.
STANDARD: The Contractor shall perform all work required by the contract in a satisfactory manner in accordance with the appropriate Performance Work Statement
(PWS) paragraph. The COR shall not consider the required contract services complete until all deficiencies have been corrected.
PROCEDURES: The COR will monitor all work required by the Contractor to ensure compliance with PWS. The COR may conduct inspections after service has been performing. The COR shall note the date and time of inspection. If inspection indicates unacceptable performance, the COR shall notify the CO. The Contractor shall receive notification from the CO to correct the unacceptable performance. The preferred method of resolving deficiencies is to allow re-performance to correct the deficiencies. The Contractor shall be provided with the opportunity to correct substandard performance within one business day. When substandard performance is not corrected within the specified timeframe or the Contractor refuses to re-perform the service, the unsatisfactory service is logged as a deficiency. Invoice deductions for deficiencies shall be calculated as one (1) day of missed service if the Contractor fails to correct.
