About this file

This document is a clinical reference guide for the State of Maine outlining evidence-based interventions (ARORA) designed to improve immunization rates in healthcare practice settings. The document provides a comprehensive framework for implementing four primary intervention strategies: assessment of immunization rates with provider feedback, provider reminders regarding patients due for immunizations, establishment of standing orders for vaccine administration, and patient reminder and recall systems. The guidance emphasizes the importance of periodic assessment of immunization rates, systematic accessibility of immunization records within patient medical records, and the implementation of feedback mechanisms to guide practice innovation and change.

The interventions outlined focus on operational and clinical process improvements rather than procurement of specific products or services. Key recommendations include ensuring that immunization records are easily accessible and systematically reviewed during every patient visit, utilizing standing orders to facilitate comprehensive vaccine administration during all visit types (well, sick, and follow-up visits), and implementing both patient reminder systems and recall mechanisms, particularly for high-risk populations with delayed immunizations or associated risk factors such as late prenatal care or missed wellness checkups. The document emphasizes avoiding false contraindications to immunization and administering all needed vaccines simultaneously to maximize efficiency and patient outcomes.

Text version

The following interventions (ARORA) are likely to be effective in any clinical setting:

Assess immunization rates and provide feedback to providers

Intervention: feedback to providers on their immunization rates A practice's immunization rates should be assessed periodically Feedback should be given to providers Use results to guide practice innovation and change

Records must be accessible with prompts for providers

Intervention: reminders for providers re: patients due for immunizations

Medical records need to be accessible.

Immunization records must be easily accessible within the patient's medical record.

Immunization records must be systematically checked on every visit to determine if any vaccines are needed.

Opportunities must be taken to vaccinate

Intervention: standing orders Providers must use all opportunities to vaccinate, including well, sick and problem follow-up visits.

Only true contraindications to immunization should be used, false contraindications must be avoided All needed vaccines must be administered simultaneously.

Standing orders may facilitate immunizations.

Reminder and Recall systems should be used

Intervention: patient reminder and recall re: when imunizations are due

Patient reminder systems should be implemented, especially for high risk patients (those who have delayed immunizations, or have risk factors associated with delays such as late prenatal care or missed well check-ups).

Patient recall systems should be established to bring back patients with delayed immunizations.

Assess immunization rates and provide feedback to providers

Intervention: feedback to providers on their immunization rates A practice's immunization rates should be assessed periodically Feedback should be given to providers Use results to guide practice innovation and change
