Water Handling vendors will be required to submit weights of each of the water tenders and engines they provide a bid for along with this check sheet. For those responsive vendors whose resources pass the documentation inspection, you will be contacted by a Fire Operations representative for physical inspection (see Exhibit M of the solicitation) starting the 2019 summer season and continuing to the end of the year. Please do not call for inspection appointments, this will delay the inspection process. If any of the documentation is illegible or decipherable, the resource/equipment will no longer be considered for award. Please disregard the D.16.1-- EQUIPMENT WEIGHT within the solicitation. At the time of inspection, engines and water tenders will be weighed twice; once when empty and once when fully loaded. Weighing of equipment at the inspection site will not be done. At this time, Region 5 will only require the weight information below.

Required documentation to be mailed:

1. A clear picture of the manufacturer’s GVWR data plate showing the GVWR for the front axle, rear axle and total gross weight rating of that piece of equipment. Pictures that do not clearly show the needed information are not acceptable and the associated resource will no longer be considered for award.

2. Weight Slips from a Certified Scale, Current (within 1 year) certified fully loaded weight receipt with the following weights:

a) Certified Gross Vehicle Weight

b) Certified Front Axle, loaded weight

c) Certified Rear Axle, loaded weight

3. Signature below certifying that the equipment being submitted meets the following at clause D.2.1.2:
