Contracting Office Address Utah Acquisition Support Center 1749 West 500 South Salt Lake City, UT 84104 Description This is a combined synopsis/solicitation for commercial services prepared in accordance with the format in Federal Acquisition Regulation (FAR) subpart 12.6, “Streamlined Procedures for Evaluation and Solicitation for Commercial Items,” as supplemented with additional information included in this notice. This announcement and attachment constitutes the only solicitation; quotations are being requested, The associated North American Industrial Classification System (NAICS) code for this procurement is 115310, with a small business size standard of $18.0 million.
The Salmon-Cobalt Ranger District, Salmon-Challis National Forest, is in need of stand exam services.
All interested companies shall provide quotation(s) for the following:
Services The United States Department of Agriculture (USDA), Forest Service (FS), Intermountain Region, The Forest Service proposes to contract for technical forestry services to conduct the field phase of a stand exam project. The stand exam will provide the Government with natural resource information for forested and other lands at an intensity required for management decisions.
The Contractor shall furnish all labor, equipment, transportation, supervision, supplies (except those designated as Government-furnished), incidentals, and perform all work necessary to conduct stand examinations in compliance with terms, specifications, conditions, and provisions of this solicitation.

The Government will award a contract resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the Government, price and other factors considered. The following factors shall be used to evaluate offers:
Factor #1 – Technical Capability of Item Offered Subfactor #1 – Ability of Item to Meet Required Specifications - In order to receive a rating of “acceptable” for this subfactor, offer must at a minimum, demonstrate adequate technical capability of the service offered to meet or exceed the technical specifications. Additionally you must receive a rating of acceptable the categories below in order to receive an overall rating of acceptable in this subfactor:

| - Adequate equipment to perform the work within specified performance period. |
| - Capability to complete work to standards (past experience, certifications, training, etc.) |
| - Safety, provide a safety history that demonstrates no safety infractions within the previous three years. |
Subfactor #2 – Past Performance - In order to receive a rating of “acceptable” for this subfactor, offeror must, at a minimum provide past experience on similar type contract(s). The past experience should indicate successes of the contractor in completing/meeting delivery dates and customer satisfaction.

The Government may also use past performance information obtained from other than the sources identified by the offeror. Information obtained from the Past Performance Information Retrieval System (PPIRS) database located at http//www.ppirs.gov is one of the sources that will be utilized. PPIRS functions as the central warehouse for performance assessment reports received from several Federal performance information collection systems and is sponsored by the DOD E-Business Office and administered by the Naval Sea Logistics Center Detachment Portsmouth.
Factor #2 – Price or Cost Evaluation factors, when combined, are approximately equal to cost or price”.
NOTE: In accordance with FAR 15.305 (a)(2)(iv)), in the case of an offeror without a record of relevant past performance or for whom information on past performance is not available or so sparse that no meaningful past performance rating can be reasonably assigned, the offeror shall be determined to have unknown past performance. In the context of acceptability/unacceptability, “unknown” will be considered “acceptable.”
PRICE: Award will be made to the lowest priced offeror that is evaluated as “acceptable” for all non-price factors.

The Offeror shall complete only paragraph (b) of this provision if the Offeror has completed the annual representations and certification electronically in the System for Award Management (SAM) accessed through https://www.sam.gov. If the Offeror has not completed the annual representations and certifications electronically, the Offeror shall complete only paragraphs (c) through (u)) of this provision.

All quotations shall be sent to the Utah Acquisition Support Center, 1749 West 500 South, Salt Lake City, UT 84104
This is an open-market combined synopsis/solicitation for services as defined herein. The government intends to award a purchase order as a result of this combined synopsis/solicitation that will include the terms and conditions set forth herein. To facilitate the award process, all quotes must include a statement regarding the terms and conditions herein as follows:
"The terms and conditions in the solicitation are acceptable to be included in the award document without modification, deletion, or addition." OR "The terms and conditions in the solicitation are acceptable to be included in the award document with the exception, deletion, or addition of the following:"
Quoter shall list exception(s) and rationale for the exception(s).
Submission shall be received not later than 09:00 A.M. MST, 5/17/2019 at 1749 W 500 S South Salt Lake City, UT 84104. Late submissions shall be treated in accordance with the solicitation provision at FAR 52.212-1(f). Faxed or e-mailed quotes will be accepted.
All quoters shall submit the following: 1 Copy of AT A MINIMUM, OFFEROR SHALL SUBMIT THE FOLLOWING DOCUMENTS BACK WITH THEIR RESPONSE TO THIS SOLICITATION:

SITE VISIT
A site visit isn’t scheduled to take place. Offerors or quoters are urged and expected to inspect the site where services are to be performed and to satisfy themselves regarding all general and local conditions that may affect the cost of contract performance, to the extent that the information is reasonably obtainable. In no event shall failure to inspect the site constitute grounds for a claim after contract award.
