Elevator Maintenance and Repair Services Region 5, Regional Office
STATEMENT OF WORK
Region 5, Pacific Southwest Region
Regional Office Elevator Maintenance Service
1. GENERAL
The Contractor shall provide all personnel, equipment, supplies, tools, material, supervision, and management to perform all routine, non-routine, preventative maintenance, and emergency repair services, in compliance with the ASME Safety Code requirements, the manufacturer’s recommendations, the Elevator Industry Field Employees’ Safety Handbook, National Electrical Code, and all other applicable laws, regulations, rules, ordinances, and codes. The Contractor shall repair, replace and/or service a piece of elevator equipment, component or part of the elevator system as is required in a well-planned preventative maintenance program. All preventative maintenance scheduled for each month must be completed during the month. The Contractor will also maintain a log for each elevator. The log shall be used to indicate all repairs, replacement of parts, and adjustments performed.
1.2 Location: All work performed under this contract will take place at the following location: USDA Forest Service, Region 5, Regional Office, located at 1323 Club Drive, CA 94592.
1.3 Work Hours
Hours of work are restricted to 7:30 a.m. – 3:00 p.m. Monday through Friday (Tuesday through Thursday preferred), unless otherwise agreed upon by authorized U.S. Forest Service representatives. Work must be performed during the regular work week based on a regular schedule approved of by the Contracting Officer’s Technical Representative (COR) of the contract.
1.4 Safety
Contractor shall require his/her employees to work within proper safety standards, practices and wear proper safety equipment required for each specific job, including the wearing of goggles, ear protection gloves and other appropriate safety equipment and protocols based on industry standards and the wellbeing of the property, building, vehicles, and staff at all times.
1.5 Building Elevator Equipment
The inventory provided herein represents the most accurate and current record of equipment and systems.

2. SCOPE OF WORK
2.1 Monthly Elevator Preventative Maintenance and Services
2.1.1 The contractor shall perform all periodic routine preventative maintenance checks and services, cleanliness, inspection, certifications, and to ensure safe and continuous operation of Regional Office Elevators in accordance with manufacture recommended services and industry standards. Service and inspections on elevators shall including but not be limited to the following: machine rooms, controller components, relay boards, terminals, fuses, bulbs, batteries, power supplies, doors, door lock circuits, elevator shafts, relays, supervisory panels, emergency telephones and connectivity, Fire Service Communications and equipment, fluid levels, temperatures and viscosities, stopping accuracy and leveling, door closure force, gate equipment, power transfer belts, linkages, rollers, hanger assemblies, landing panels, clutches, door gibs, gib fasteners, stop switches, signal buttons, alarms, service lighting, emergency lighting, alarms, ventilation fans, car top lighting, stop switches, hoist ways, tracks, guide shoes/roller guides, rollers, door roller hanger assemblies, springs, leveling devices, and elevator pits.
2.1.2 The contractor shall ensure that the elevator pits, interior, and exterior elevator cars, mechanical rooms, and all associated equipment is free of debris, visible dirt, fluid residue, and properly lit. The contractor shall ensure that all mechanical moving parts and components are properly lubricated to ensure safe, smooth, and friction-free operation.
2.1.3 The Contractor shall schedule and/or perform all Hydraulic Elevator Periodic Inspections, Testing and Certification to meet safety requirements and industry standards including but not limited to:
a) The Contractor shall schedule and have performed the tests of all elevators at the recommended intervals as outlined by manufacturer recommendations and required by law. Semi Annual Certification tests and inspection shall be performed by an independent state licensed elevator inspector in accordance with ASME and Industry Standards. Certificates of Inspection shall be mounted on the interior of each elevator car in designated certificate location.
1. The Contractor shall schedule semi-annual inspections and annual testing no less than 30 days prior to expiration of current inspection or testing tag. The Contractor shall notify the COR of the scheduled inspection or test 30 days prior to the inspection. The Contractor or qualified representative shall be present during the entire inspection(s) process with the inspecting official.
b) Conduct Annual Hydraulic Elevator Testing (Category 1: No Load Testing) in accordance with ASME Standard Requirements and State of California Standards for Elevator Safety.
c) Conduct Periodic 5-Year Category 5: (Full Load Testing) in accordance with ASME Standard Requirements and State of California standards for elevator safety as required.
d) Complete Periodic Test Record Tags to include but not limited to ASME Federal and State Inspection Tags and attached as required.
e) Provide Record of Testing and all deficiencies identified to COR as required.
2.1.4. The Contractor shall maintain monthly maintenance logbooks for each elevator. The maintenance logbook shall be kept in the elevator rooms. Maintenance logbooks will contain at minimum the elevator maintenance manual, service inspection tasks performed for each visit maintenance visit, the date each task was performed, and the servicing technician’s initials.
2.1.5 Reprogram Emergency Telephone Line in Elevator 1 and Elevator 2. The Contractor shall program the autodial ATT emergency telephone lines in Elevator 1 and Elevator 2 within five (5) business days.
Programming shall include the building’s elevator car and location and shall automatically dial outreach the Contractor’s emergency 24-hour monitoring and dispatch line. The Contractor shall coordinate and maintain emergency line connections. The Contractor shall response to all emergency calls within two (2) hours of calls made from the emergency phone lines.

3. ADDITIONAL WORK REQUIREMENTS:
The Contractor will provide all necessary supervision, material, labor, and equipment to complete the following deficiencies as part of the base contract:
3.1 Car 1- Provide proper operation of removed car door restrictor.
3.2 Car 1 – Replace emergency battery pack on top of car.
3.3 Car 1 – Repair inoperative in car emergency phone.
4. NON-ROUTINE AND EMERGENCY REPAIRS/SERVICE CALLS
4.1 Non-Routine Services: A non-routine service call is defined as any maintenance that is not performed at pre-determined intervals. Non-routine work includes repairs, parts replacement, and services that address problems found during routine maintenance, inspections, annual maintenance, normal operation, or while performing routine services and preventative maintenance. Non-routine repair(s) and/or part(s) replacement shall be completed, and all work shall be performed during the normal work hours as identified in this contract. The Contractor shall report to the work location, survey the repair, and provide to the COR/KO an estimated time to repair, estimated downtime of equipment, and the cost of repairs prior to starting work. The Contractor shall commence repair work after notification from the COR. All repair work shall be in accordance with normal commercial practices using parts specified by the elevator manufacturer or items of equal or better quality. The Contractor shall prepare and submit to the Government a detailed written report within two (2) calendar days after the non-routine repairs are completed. The report shall include the date and time of the service call, the location of the elevator, the repairs performed, warranty information of replaced parts/repairs and the name of the technician performing the repairs.
4.2 Emergency Calls: The Contractor shall respond to emergency repair calls within two (2) hours from the time of notification during normal business hours and within four (4) hours from the time of notification at times other than normal business hours that the elevators and auto doors are out of service. A call of this type would suggest that there are trapped persons, or the elevator and auto door condition could result in life, health, safety, or property endangerment, or could have an adverse effect upon personnel, equipment, and mission accomplishment. Emergency service shall be available 24 hours a day, seven (7) days a week. If this type of call requires emergency repair, the Contractor shall prepare and submit to the Government a detailed written report within two (2) calendar days after the emergency repair. The report shall include the date and time of the service call, the location of the elevator, the repairs performed, and the name of the technician performing the repairs.
5. CONTRACTOR REQUIRED DELIVERABLES
The Contractor shall provide all necessary supervision, material, tools, equipment, and labor to complete all requirements to perform work under this contract.
5.1 The Contractor shall provide a schedule/timeline for scheduling and completion of all work at pre-work meeting.
5.2 The Contractor shall provide to the COR a maintenance plan and schedule within 14 days after award that identifies when monthly maintenance and periodic testing and certification will occur.
5.3 The Contractor shall provide a monthly maintenance and inspection checklist for each elevator that identifies preventative maintenance checks and services to be completed, technician name and dates each task is completed. The checklist shall be kept in a clear plastic, waterproof sleeve and attached in a visible location in each mechanical room.
5.4 The Contractor shall provide and maintain an Elevator PM Logbook for each elevator that meets ASME Code for proper contents. Logbooks shall be in each mechanical room.
5.5 The Contractor shall provide a qualified employee to accompany the inspector during annual testing. The contractor will provide all record forms and certifications for annual testing within five (5) business days of completion. All deficiencies found during testing will be documented and provided to the COR within five
(5) business days. The Contractor will provide a quote for all deficiencies found to the COR within five (5) business days.
6. GOVERNMENT FURNISHED REOURCES
The Government shall provide access to building, elevators, utilities, mechanical rooms, electrical and telephone connections to allow the contractor to perform work under this contract.
7. PAYMENT
Contractor shall submit invoices via the Invoice Processing Platform (IPP) ipp.gov; invoices shall be submitted monthly after services have been performed (arrears).
