About this file

This document is a Schedule of Items for a Request for Quote (RFQ) for the Monongahela National Forest FR704 and FR705 Emergency Road Failure (ERFO) Road Project in Tucker County, West Virginia. The project involves road infrastructure improvements with an estimated performance period from October 20, 2025 to June 15, 2026. The solicitation (No. 12445525Q0103) requires comprehensive road and drainage improvements, including soil and erosion control, turf establishment, excavation, clearing, dewatering, riprap placement, aggregate placement, road reconditioning, and culvert installation.

The procurement is structured as a Firm Fixed Price (FFP) purchase order with 16 line items covering various construction activities. Key requirements include active SAM.gov registration, submission of technical quotes via email to specified contacts, and a mandatory site visit scheduled for September 30. The project is issued by the Department of Agriculture Forest Service, with a total budget implied by the line item pricing structure. Contractors must provide unit pricing for each line item and are advised that work not explicitly listed is considered incidental to the primary scope of work.

SCHEDULE NOTES

1) Abbreviations: LS = Lump Sum, FT = Feet, LF = Linear Feet, SY = Square Yard, CY = Cubic yard, AQ = Actual Quantity, CQ = Contract Quantity, DQ = Design Quantity.

2) One Firm Fixed Price (FFP) purchase order is anticipated from this RFQ. Contractors must enter pricing for all line items (both unit prices, and total amounts).

3) Type prices directly into the schedule provided to promote readability.

4) Any purchase order awarded from this RFQ that exceeds $35,000.00 will require payment protections IAW FAR 52.228-13.

5) The price of any applicable bonds is considered incidental to the purchase order as a whole, and is included in the total award price. The award price of any purchase order that results from this RFQ will not be increased to accommodate a contractor’s failure to account for bonds in their quote.

6) Payment will only be made for the items listed in the schedule above. Any work needed to complete this project that is not listed in the schedule is considered incidental to the items shown. This statement is NOT AN INVIDTATION TO ADD ADDITIONAL LINE ITEMS. If contractors wish to recommend alternative line-item structures, follow the procedures in FAR provision 52.204-22 Alternative Line-Item Proposal. If you believe a needed line item is incorrectly omitted from the existing schedule, contact paul.boucher@usda.gov, and if necessary an amendment will be issued to update the schedule of items for all contractors.

7) Contractor must have an active registration at sam.gov to be eligible for award. A status of “ID Assigned” is not a fully activated registration.

8) The SAM Unique Entity ID (UEI) is a string of 12 letters and numbers. Do NOT provide a 9-digit DUNS. The 12-character UEI is the official designator, and has replaced the DUNS field in all contracting websites and databases.

9) This RFQ contains technical evaluation factors other than price. Refer to L.2 Preparation Instructions for details on requirements for your quote.

10) A formal site visit has been scheduled for September 30. Refer to provision FAR 52.236-27 Site Visit (Construction) for details.

11) Send technical questions to paul.boucher@usda.gov and leo.weese@usda.gov. (Include both email addresses).

12) Completed quotes are to be emailed only to paul.boucher@usda.gov.
