About this file

This document is a Past Performance Questionnaire (PPQ) that requests a reference from a client for a contractor's past performance on a federal contract. The PPQ includes sections for the contractor's company information, the client/reference information, and an evaluation of the contractor's performance on 11 specific criteria, such as providing experienced managers, quality control, problem solving, and cooperation with government personnel. The client is asked to rate the contractor's performance on a scale of 1 to 5, with 5 being "Exceptional." The client is also asked whether the contractor was ever issued a cure or show cause notice, and whether the client would award another contract to the contractor. The PPQ is related to a federal contract opportunity for Janitorial Services for the USDA Forest Service in Nacogdoches, Texas.
