About this file

This is a Past and Present Performance Questionnaire for a federal contract solicitation. Solicitation Number 12444826Q0006 concerns Hamden Janitorial Services and is being used to evaluate contractor past performance as part of the source selection process. The questionnaire is designated as source selection sensitive under FAR 2.101 and 3.104. Respondents must complete Section A (General Information) by providing or correcting the contractor's name, address, telephone number, project title, contract number, dollar amount, contract period, and the contractor's role (Prime Contractor, Sub-Contractor, or Key Personnel). Section B requires respondent information including name, title, telephone number, and email address. Completed questionnaires must be returned by COB February 8th, 2026 at 2359 ET via email to Christopher Hamilton at christopher.j.hamilton@usda.gov.

Section C contains the performance evaluation criteria using a 1 to 5 rating scale, where 1 represents Unsatisfactory, 2 represents Marginal, 3 represents Satisfactory, 4 represents Very Good, and 5 represents Exceptional, with N/A available where appropriate. Evaluators must rate the contractor on nine performance factors including management capability, problem-solving, quality control, timely deficiency correction, problem identification, alternative approaches, initiative, Government cooperation, and overall performance. Three yes/no questions address whether the contractor received a cure or show cause notice, whether the contractor is rated in CPARS, and whether the evaluator would award another contract to the contractor. Narrative explanations are required for any ratings of 1 or 2. The questionnaire includes space for remarks and requires identification of the contractor's name and contract number.
