a) One award will be made from this solicitation, contractors must submit pricing for all items.
b) Quantities shown are estimates only. Invoicing and payment based on actual production hours.
c) Invoicing will be based on actuals as they occur. Invoice to the 15-minute interval. Two (2) hours and ten (10) minutes would be invoiced as 2.25 hours.
d) Invoices will require supporting documentation for Labor Hours worked.
Description of Requirement Furnish all labor, supervision, equipment, supplies, and transportation necessary to provide snowplowing services as per the attached Statement of Work.

(d) Comptroller General Examination of Record. 52.217-8 Option to Extend Services (Nov 1999)
The Government may require continued performance of any services within the limits and at the rates specified in the contract. These rates may be adjusted only as a result of revisions to prevailing labor rates provided by the Secretary of Labor. The option provision may be exercised more than once, but the total extension of performance hereunder shall not exceed 6 months. The Contracting Officer may exercise the option by written notice to the Contractor within 14 days.
52.217-9 Option to Extend the Term of the Contract (Mar 2000)
(a)The Government may extend the term of this contract by written notice to the Contractor within 7 calendar days; provided that the Government gives the Contractor a preliminary written notice of its intent to extend at least 14 calander days before the contract expires. The preliminary notice does not commit the Government to an extension.
(b)If the Government exercises this option, the extended contract shall be considered to include this option clause.
(c)The total duration of this contract, including the exercise of any options under this clause, shall not exceed 5 years and 6 months.
452.204-70 Modification for Contract Closeout (DEVIATION JUL 2022)
Upon contract closeout for contracts utilizing Simplified Acquisition Procedures (SAP) according to FAR 13:
(a) If unobligated funds in the amount of $1000 or less remain on the contract, the Contracting Officer (CO) shall issue a unilateral modification for deobligation. The contractor will receive a copy of the modification but will not be required to provide a signature. The CO shall immediately proceed with contract closeout upon completion of the period of performance, receipt and acceptance of supplies or services, and final payment.
(b) If unobligated funds of more than $1000 remain on the contract, the CO shall issue a bilateral modification for deobligation. The contractor will receive a copy of the modification and will be required to provide a signature. (The CO may also request a Release of Claims be completed by the contractor, although not required for contract and orders using SAP procedures.) If the bilateral modification and Release of Claims are not returned to the CO within 60 days, the CO shall release the modification as unilateral and proceed with contract closeout upon completion of the period of performance, receipt and acceptance of supplies or services, and final payment.

452.237-74 Key Personnel (FEB 1988)
(a) The Contractor shall assign to this contract the following key personnel: Supervisor.
(b) During the first ninety (90) days of performance, the Contractor shall make no substitutions of key personnel unless the substitution is necessitated by illness, death, or termination of employment. The Contractor shall notify the Contracting Officer within 15 calendar days after the occurrence of any of these events and provide the information required by paragraph (c) below. After the initial 90-day period, the Contractor shall submit the information required by paragraph (c) to the Contracting Officer at least 15 days prior to making any permanent substitutions.
(c) The Contractor shall provide a detailed explanation of the circumstances necessitating the proposed substitutions, complete resumes for the proposed substitutes, and any additional information requested by the Contracting Officer. Proposed substitutes should have comparable qualifications to those of the persons being replaced. The Contracting Officer will notify the Contractor within 15 calendar days after receipt of all required information of the decision on substitutions. The contract will be modified to reflect any approved changes of key personnel.

52.212-2 Evaluation – Commercial Products and Commercial Services (NOV 2021)
(a) The Government will award a contract resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the Government, price and other factors considered.

(b) Options. The Government will evaluate offers for award purposes by adding the total price for all options to the total price for the basic requirement. As part of the price evaluation, the Government will evaluate its option to extend services (see FAR Clause 52.217-8) by adding one half of the quoter's final option period price (base and four option periods). (c) A written notice of award or acceptance of an offer, mailed or otherwise furnished to the successful offeror within the time for acceptance specified in the offer, shall result in a binding contract without further action by either party. Project Snow Plowing and Ice Removing Services for Ansonia and Hamden, CT locations.
General Information
1.0 Scope of Work
Furnish all labor, supervision, equipment, supplies, and transportation necessary to provide snowplowing at the following locations:
U.S. Forest Service U.S. Forest Service 119 ½ Ford Street 51 Mill Pond Road Ansonia, CT 06401 Hamden, CT 06514
2.0 Background
The U.S. Forest Service requires services as follows:
Ansonia
• Contractor must remove the snow from the access road off Ford Street to the facility, off the parking lot and driveways (including a path to the Regional Water Authority 90 ft water tank), and from the entry walkways for the three buildings by the following morning prior to 7:00 a.m. (10:00 am on Sundays) after each cumulative snow fall of 2.5” or more and during the day after 3” or additional snowfalls as needed, Monday thru Sunday.
Snow shall be plowed to the designated markers located on the property and not obstruct any walkways.
Hamden
• Contractor must remove the snow from the facility driveway, off the upper and lower parking lots and from the entry walkways (including the loading dock) for the two buildings by the following morning prior to 7:00 a.m.
(10:00 am on Sundays) after each cumulative snow fall of 2.5” or more and during the day after 3” or additional snowfalls as needed, Monday thru Sunday. Snow shall be plowed to the designated markers located on the property and not obstruct any walkways.

3.0 Technical Requirements / Tasks
• Furnish all labor, supervision, equipment, supplies, and transportation necessary to provide snowplowing and ice melt service at the U.S. Forest Service locations of Ansonia & Hamden, CT.
• All vendors are highly encouraged to coordinate and schedule a Site Visit / facility inspection prior to submitting their offer.

• Awardee will have a MANDATORY Post Award Meeting prior to the start of the performance period.
• The exact service dates shall be coordinated with the location (Ansonia and Hamden) Forest Service staff.
• Work shall not exceed the amount of the award without written approval from a Warranted Contracting Officer.
• Invoices must be detailed and uploaded into IPP.gov in a timely manner based on services actually rendered.
• Offerors MUST have an active entity registration in the System for Award Management in order to submit an offer. https://www.sam.gov/SAM/
• One award will be made from this solicitation, contractors must submit pricing for all items.

Ansonia Snow/Ice Removal Services
Driveways and Parking Lots:
• Snow shall be removed from the access road off Ford Street to the facility, off the parking lot and driveways
(including a path to the Regional Water Authority 90 ft water tank), and from the entry walkways for the three buildings.
• Service shall be provided before 7:00 AM, Monday through Saturday, and by 10:00 AM time frame for Sunday for ≥ 2.5” cumulative snow.
• Snow falls of ≥ 3 inches will be plowed as needed when snow continues to fall throughout the day. When additional service is required because of continuing snow fall, the POC will contact the contractor, and this will be treated as a separate service call.
• Care shall be taken so that no damage occurs to parked vehicles and existing structures.
Walkways, sidewalks, building entrances:
• Shovel/blow snow away from sidewalk and building entrances.
• Service shall be provided as indicated for plowing.
Road and Parking Lot Sanding/Salting:
• The access road, walkways and parking area should be salted when surface conditions warrant this, such as icing.
• Parking areas should be salted/sanded when icing exists.
• Sand/salt shall be applied only by the request of the Contracting Officer Representative (COR) or Director’s
Representative for the facility (Melody Keena 203-260-1984). They will err on the side of making conditions as safe as possible.
