Performance Work Statement
Project Title San Dimas Epoxy Flooring Project
General Information
1.0 Background and Purpose
The US Forest Service's National Technology and Development Program (NTDP) facility, located in San Dimas, CA, has a requirement to replace approximately 1,100 sq ft of existing flooring with epoxy flooring as follows:
· T-10 carpeted office approx.34'x11'
· T-17 carpeted office approx.24'/4"x16'
· D-3 Pubs room concrete flooring approx. 16’/2”x18’/9”) converted to Epoxy flooring, transforming these rooms to Laboratories and work shops helping extend the facilities capacity for Research and Development.

Contractor Requirements
3.0 Technical Requirements / Tasks
· The contractors shall be responsible for the removal and cleanup of all old/existing floor covering as well as the sealing of any cracks or breaks and correction to any floor imperfections or deformities in preparation to Epoxy slurry applications. Along with all floors, all 3 areas will need Epoxy applications to be applied 6" up ALL perimeter walls as a cove base. All applications shall have a moisture mitigating primer as well as an application of moisture barrier.
· For Interior office space T-17, the contractor shall apply an application similar or equivalent to: a solid color, seamless flooring system consisting of 100% solids epoxy designed for coating industrial and commercial floors. A durable urethane topcoat ensures excellent wear characteristics, making it a great system for varying areas from light manufacturing to clean rooms. (Color: Grey) To be approved by the COR.
· For Interior office space T-10, the contractor shall apply an application similar or equivalent to: a seamless, decorative flooring system consisting of 100% solids epoxy body coats with decorative vinyl chip broadcasts. A durable urethane topcoat ensures excellent wear and chemical resistance. Available in standard and custom color blends in both micro and macro sizes. (Color: Oyster Shell) To be approved by the COR.
· For Interior office space D-3 the contractor shall apply an application similar or equivalent to: Shop Floor, a seamless, slip resistant pigmented flooring system consisting of 100% solids epoxy and natural quartz aggregates. Easy to maintain and durable, Shop Floor that is ideal for industrial flooring applications and features a chemical, UV, and wear resistant urethane topcoat. “This area will need to be to endure Heavy Equipment Traffic such as (“Skid steers, Forklifts, Pallet jacks, etc.”) (Color: Grey). To be approved by the COR.

5.0 Government Furnished
· Electricity (120V Outlets), water (On Site water Fountains), and sanitary facilities (1 Designated restroom, that contractor shall be responsible to clean up behind themselves after each use) at no cost to the contractor.
6.0 Travel
· To/From the work site, travel expenses are the responsibility of the contractor. No other travel is anticipated.
7.0 Contractor’s Key Personnel
· Experienced field personnel are expected. Contractor shall provide the name and contact of a on-site
POC.
8.0 Security Requirements
· All contractor personnel must sign in at the front desk and wear visible USDA Visitor Identification.
· Adhere to CDC guidelines for face coverings and other health and safety protocols.
· NTDP Center Access hours will be as follows: M-F 7:30A – 4:30P Excluding Holidays.
9.0 Performance Requirement Measures
· NTDP Center Access Hours will be as follows: M-F 07:30AM - 4:30PM, excluding holidays.
· The Government will measure the contractor’s performance for work critical to the mission essential items.

Ensure Epoxy applications meet 3.0 of this contract’s standards for all 3 designated areas.
Ensure all Epoxy applications are properly layered to meet each locations intended use. Defining the responsibilities, timeline, risks and milestones of contract objectives No later than five (5) business days after contract award.
Report documenting tasks & issues weekly. Weekly, every Thursday, by 3:00 PM PST until the contract expires Epoxy application to be cured, clean, safe, sanitary and able to be fully utilized and be accessed by Government 45 days after contract award.
