About this file
This is a Quality Assurance Surveillance Plan (QASP) attachment for a Willamette National Forest tree planting contract in the Detroit Ranger District for 2026 (Contract Number 1240BE26Q0045).
The document establishes quality control and inspection requirements for both the contractor and government. The contractor must submit a Quality Control Plan (QCP) demonstrating how quality will be monitored, work supervised, results tracked, and personnel assigned. The contractor is responsible for conducting planting quality assurance inspections using 1/50th acre sample plots at a minimum intensity of one plot per two acres (1% sample intensity), with a minimum of three plots required for all units regardless of size. Inspections must evaluate above-ground and below-ground planting quality including plantable spots, planted tree count, species mix, micrositing, site preparation, planting depth, stem position, spacing, tree damage, and firmness. Quality is calculated using the formula: (Number of Satisfactory Trees Above-Ground ÷ Number of Planted Trees) × (Number of Satisfactory Dug Trees ÷ Number of Dug Trees) × 100%. The government will conduct its own independent quality assurance plots using the same inspection methodology on different plot locations and sample intensities. Results must fall within 5% of each other for acceptance, and all inspection sheets must be submitted the following morning after inspection.
The contract establishes minimum acceptable quality at 80 percent, with a threshold for contractor discretionary replanting at 90 percent. If quality falls below 80 percent, the contractor must replant (if seedling stock is available) at no cost to the government, with only one replant permitted and the contractor bearing reinspection costs. Payment is deducted for quality below 90 percent. The document prohibits contractors from falsifying inspection results or influencing planting quality, with potential criminal prosecution under 18 U.S.C. § 1001 or § 287. If satisfactory work falls below 80 percent, the government will issue written notice and may terminate the contract if quality is not raised above 80 percent within two consecutive workdays.

QUALITY ASSURANCE SURVEILLANCE PLAN
Willamette NF, Detroit RD 2026 Tree Planting
CONTRACTOR QUALITY CONTROL PLAN
A. Quality Control Plan
The Contractor shall provide a Quality Control Plan (QCP) as part of this Technical Proposal. The Plan must demonstrate how the Contractor shall assure that quality of performance meets the objectives and requirements of the contract. The QCP shall specify:
- How quality will be monitored to ensure performance standards are met.
- How the work will be supervised.
- How results of the monitoring will result in quality performance.
- Identify the personnel responsible for performing quality control.
B. Contractor Quality Control Inspection
The Contractor shall assure themselves that performance meets contract specifications, in accordance with FAR 52.212-4, Contract Terms and Conditions -- Commercial Items, paragraph (a), prior to requesting the Government to inspect for payment or acceptance. The Contractor shall contact the Contracting Officer upon completion of a work area. The Contracting Officer may observe the Contractor’s inspection at any time and shall otherwise have unlimited access to the inspection data.
ADDITIONAL INSPECTION AND ACCEPTANCE REQUIREMENTS
The Contractor may be put on notice of non-compliance (or other contract action taken) for failure to conduct plot inspections in accordance with the QCP. The Contractor’s inspector is not permitted to modify plot deficiencies nor influence the planting quality by indicating upcoming plot locations to the planters.
In the event the Contractor falsifies inspection results as determined by the Contracting Officer, the Government may declare the Contractor to be in default of the contract. Additionally, the Government may refer such falsification to the U. S. Attorney’s Office for possible prosecution pursuant to 18 U.S.C. 1001 (Falsification of Government Documents) or 18 U.S.C. 287 (Fraud).
CONTRACTOR INSPECTION PROCEDURES
Planting Quality Assurance Plots: the Contractor will be required to take representative sample plots (at 1/50th acre) of the unit to verify planting quality both above ground and below ground using the Planting Inspection Instructions (Exhibit F) and will record results using the Planting Inspection Form (Exhibit G):
1. Determine total number of plots required for each unit based on a minimum of one 1/50th acre plot per two acres of planting work (1% sample intensity). A minimum of 3 plots is required for all units, regardless of size.
2. Establish plot centers with easy to see flagging labeled with plot number, date, and initials.
3. Determine slope percent and adjust plot radius accordingly.
4. Determine plantable and unplantable spots, number of planted trees, species mix, micrositing, site preparation, planting depth, stem position, spacing, damage to trees, and firmness. Check for contract compliance.
5. Determine number of trees to inspect underground and dig for root placement and other underground requirements.

6. Calculate quality for individual planting units based on the following formula. Calculations are based on planted trees. Under no circumstances will quality be averaged among multiple items or sub-items.
Percent Quality =
(End of clause)
GOVERNMENT QUALITY ASSURANCE SURVEILLANCE PLAN
The Government will monitor progress to assure the Contractor is providing the quality promised in their Quality Control Plan (QCP) and work meets performance standards. Surveillance will consist of visual observations of the entire operation and sample plots to determine compliance with the specifications. Plots will be located throughout the planting area to obtain a representative sample of the work. The Contractor may observe plot procedures while they are underway.
Planting Quality Assurance Plots: The Government will take representative sample plots of the unit to verify the Contractor’s planting quality for above ground and below ground using the R-6 Standard plot procedure described in the Planting Inspection Instructions (Exhibit F). The Contractor’s Quality Control Plan plot method shall mirror the Government’s plot method but be in different locations and consist of different sample intensities. The Planting Inspection Sheet, Exhibit G will be used.
The Government inspector will take enough plots to assure the Government’s quality assurance plots represent planting (adequate to judge if the Contractor’s inspection plots are within 5% of the Government’s) and if planting is acceptable. The Government size sample will be 1/50th acre plots.
The following data will be collected on each plot:
Government Inspection Procedures:
1. Determine total number of plots required for each subitem.
2. Fill in all necessary information on plot cards.
3. Establish random plot centers with easy to see flagging with plot #, date and initials.
4. Determine slope percent and adjust plot radius accordingly.
5. Determine plantable, unplantable spots, number of planted trees, species mix, micrositing, site preparation, planting depth, stem position, spacing, damage to trees and firmness. Check for contract compliance.
6. Determine number of trees to inspect underground and dig for root placement and other underground requirements.
7. Do calculations to determine percent planting for each plot inspected.
8. Inform foreman of planting crew, as soon as possible, of any concerns or deficiencies on each plot taken.
Determine where problems may exist on crews work and take either sample or count plots on those planters in question until problems are corrected.
9. Inform Contracting Officer of any concerns or deficiencies as work progresses or as soon thereafter as possible.
10. Keep records of inspections on file. Government inspections may be done concurrent with work, or as soon thereafter as possible.
The Contractor may be put on notice of non-compliance (or other contract action taken) for failure to conduct plot inspections in accordance with the QCP. The Contractor’s inspector is not permitted to modify plot deficiencies nor influence the planting quality by indicating to the planters of upcoming plot locations. The Government will review all plot inspection sheets and check calculations after receipt. All plot inspection sheets are to be turned in no later than the following morning. The Government will compare the Contractor’s plot results with the Government’s results. The Contractor will be informed if the quality is acceptable after comparing the Government assurance plots with the Contractor’s plots upon completion of entire unit. When inspection results are within 5%, the

Contractor will be informed that its planting quality is acceptable and pay will be calculated based on the Contractor’s plot results.
In the event the Contractor falsifies inspection results, as determined by the Contracting Officer, the Government may declare the Contractor to be in default of the contract. Additionally, the Government may refer such falsification to the U. S. Attorney's Office for possible prosecution pursuant to 18 U.S.C. 1001 (Falsification of Government Documents) or 18 U.S.C. 287 (Fraud).
UNSATISFACTORY PLANTING
A. If the percentage of planting quality for any sub item/unit falls below 90 percent, the Contractor may be permitted to replant the sub item/unit in order to achieve a higher planting quality percentage.
Replanting will be subject to availability of seedling stock and shall be requested in writing by the Contractor. Following completion of replanting, a new inspection shall be made by the Contractor.
Payment will be based on the new inspection.
B. The Contractor shall replant any sub item where the average planting quality falls below 80 percent, provided that:
1. Additional supplies of suitable seedlings are available,
2. Only one replant will be permitted. Payment will be based on a new inspection made after replanting. The Contractor shall bear the cost of all reinspections after replanting.
Quality below 90% will be deducted from payment (See Section 5.2.6 Payment). When inspection results are below 90 percent, and excess trees constitute any part of the deficiency, payment will not be made until the deficiency has been corrected by the Contractor. Note: Minimum acceptance quality is 80%. Reinspections, after rework, will be made in the same manner as the first inspection, but on different plot lines. The Contractor shall pay for inspections necessitated by the rework.
Reinspection of Planting upon Contractor Request
If original inspection results are unacceptable to Contractor and a second inspection is requested without rework, the same inspection procedure will be used following a different plotline. If the second inspection shows less than 5 percent variance from the first inspection, the Contractor shall pay the cost of the second inspection, and results of the second inspection will be used in determining payment. reinspection requests shall be in writing and submitted within three (3) calendar days of the original inspection notification.
UNSATISFACTORY PERFORMANCE
If the percentage of satisfactory work falls below 80 percent, the Government will immediately notify the Contractor in writing and order improvement of the quality of future work. If the quality of future work is not raised above the 80 percent within two consecutive workdays after receipt of notice in writing of unsatisfactory work, the Contractor's right to proceed may be terminated and the contract considered in default. Repeated failure to perform work at or above 80 percent acceptable performance shall also be considered reason for contract termination and default action.
(End of clause)

REWORK AND REINSPECTION AFTER REWORK
When inspection results are below 80 percent and the deficiency is correctable, payment will not be made until the deficiency has been corrected. Reinspection after rework will be made in the same manner as the first inspection but on different plot lines. The Contractor shall pay for inspections necessitated by the rework.
SUSPENSIONS OF WORK FOR OTHER THAN GOVERNMENT'S CONVENIENCE
The Contracting Officer may issue orders to suspend the work wholly or in part for such period of time as deemed necessary because of: (1) weather or ground conditions when further prosecution of the work might cause environmental or resource damage to the project, access roads to the project, or adjacent property. Such action would include but not be limited to instances such as siltation of streams, damage to access roads, rutting of project roads which causes otherwise suitable soils to be muddy or unsuitable; or (2) failure of the Contractor to comply with specifications such as but not limited to placement of frozen material in fills, placing of asphalts at temperatures lower than those specified, performing work prior to prerequisite approvals, operating equipment not meeting fire requirements, or when conditions exist which do not meet safety requirements. Whether or not a suspend work notice is issued, the Contractor shall be responsible for correcting any damage caused by its operation, whether inside or outside project limits, at no cost to the Government. Such suspensions shall not be considered as suspensions for the Convenience of the Government under FAR 52.242-15, Stop-Work Order, and shall not qualify for an equitable adjustment.
