About this file
This is a template checklist form designed to ensure offeror compliance with Federal Acquisition Regulation (FAR) 52.219-14 regarding limitations on subcontracting for small business contracts. The checklist is mandatory for all offerors, and incomplete responses may render a quote ineligible for award.
The form requires offerors to disclose their performance approach by indicating whether the prime contractor will self-perform the required work or subcontract a portion of it. If subcontracting is planned, offerors must complete detailed sections identifying each subcontractor's legal business name, Unique Entity ID, assigned NAICS code, size representation under that NAICS code, and small business program status (including certifications such as SDVOSB, WOSB/EDWOSB, HUBZone, or 8(a)). The prime contractor must provide three compliance certifications: that subcontracting will not exceed allowable percentages for non-similarly situated firms, that all listed subcontractors have provided appropriate size and program-status representations with reasonable reliance basis, and acknowledgment that non-compliance may result in termination for default, negative CPARS ratings, or other government remedies. The form concludes with signature blocks for an authorized official and optional space for clarifying remarks regarding the performance approach or subcontracting strategy.
