14. DESCRIPTION OF AMENDMENT/MODIFICATION
Project Name and Location: MASON DAM BOAT LAUNCH IMPROVEMENTS This amendment addresses the following (see attached addendum)

By signature in Block 15B Contractor releases the Government from all claims relating to this modification. This settlement is complete accord and satisfaction. Except as provided herein, all terms and conditions of the document referenced in Item 9A or 10A, as heretofore changed, remains unchanged and in full force and effect.

Addendum to Amendment 1
REVISIONS, PRE-BID INQUIRIES, REQUEST FOR INFORMATION MASON DAM BOAT LAUNCH

Section L-11, 3 (ii)
1 original and 3 copies of the technical proposal are required for evaluation.
Revision-002
Revised Section B – Supplies or Services and Prices: Line item 10, Subbase for CIP (3”-0”) and Line Item 11, Aggregate base for CIP (3”-0”) are revised as follows: Line item 10, Subbase for CIP (2 1/2”-0”) and Line Item 11, Aggregate base for CIP (3/4” minus). (See attached bidder’s package for revised bid schedule).

Compaction of subgrade: Section 02200-3.2-D specifies “Subgrade shall be thoroughly compacted by a minimum of two passes with a mechanical type compactor to obtain 95% of maximum density to a depth of 1 foot below established subgrade elevation.” Because of the impracticality of obtaining a reference proctor density in varying subgrade material, testing to meet this specification will be waived. Inspection for compliance with this section will be visual observation of the subgrade behavior during compaction. A minimum of 2 passes with a mechanical compactor will be required, repeated passes may be required until the subbase is observed to not yield under further compaction.
Revision-004
Section H-11 Add -Use of FS Vault toilet by contractor will be allowed in lieu of providing portable toilets. Contractor shall clean the toilet building floor, interior, and pump the vault, if needed, at completion of the job.
Pre-Bid Inquiries (PBI’s) for Contract Bidding
PBI-001
Question: Can FS provide a section drawing of existing asphalt?
Answer: No
PBI-002
Question: How do contractors deal with Section 03360-3.1/D placement of concrete in temps below 40 degrees?
Answer: Placement of concrete at temperatures of 40 degrees and below will be in accordance with cold weather concrete procedures proposed by contractor and approved by CO. Use of heating devices, blankets, and similar provisions to maintain temperatures as specified in Section 03360-3.1 D will be required to meet this requirement.
PBI-003
Question: Does contractor need to have material certifications for all base, subbase, and riprap, how about shop drawings for rebar?
Answer: Yes- gradation for mineral material and concrete testing reports required Section 03360-4.1 and 4.2 No- rebar shop drawings will not be required.
PBI-004
Question: Is there a specific area on-site the FS has desingated for construction staging?
Answer: Contractor will be expected to close the entire access road, parking lot, and site to the public for the duration of the work.
The asphalt parking lot and gravel parking area half way down the ramp are available for staging. Grading of the area adjacent to the ramp on each side to allow access and to facilitate construction and concrete placement will be allowed; restorative grading at completion will be required. Erosion control measures will need to be adapted to any access and staging areas disturbance.
PBI-005
Question: Is there a specific area the FS would prefer concrete washout station?
Answer: Contractor shall be allowed to wash out concrete trucks on site. Excavation of a shallow pit to contain wash water until it infiltrates into subsoil will be required; erosion control wattles downstream of the site to further insure against runoff water contamination will also be required. Location of washout area shall be proposed by contractor and approved by COR prior to construction.
PBI-006
Question: Can contactors re-use material onsite?
Answer: Salvage, stockpiling and re-use of granular or soil subgrade material will be considered if the material meets project specifications. COR will also evaluate use of the excavated material for access area construction, disposal on existing gravel parking lot, and other on-site uses in lieu of haul off and disposal. Asphalt and concrete debris will be removed from site and disposed of by Contractor
PBI-007
Question: Are permits required?
Answer: Section 01019-1.1 states that USACE and ODSL permits are pending. USACE and ODSL have determined no permits are required for this project. No other permits are required for on-site project work. Contractor shall be responsible for any overload or other off-site permits required for their operations.

Question: Is re-seeding the project site required?
Answer: No
PBI-009
Question: When will Notice to Proceed be issued?
Answer: Depends on the bids and proposals. If no negotiations are required, then a very quick turnaround for starting work is expected due to the aggressive schedule.

Question: Is there a FS pit that can be used for disposal?
Answer: No
PBI-010
Question: How open is FS to pouring concrete in frozen conditions?
Answer: If gravel is dry and compacted prior to pouring, and no accumulations of ice buildup, then approval would be given by COR to pour.
