About this file

This is a Vendor Certification form from the Harris County Juvenile Probation Department related to a Request for Sealed Quotation (RFSQ) for Residential Services in Texas, with a contract term beginning approximately June 1, 2025, for one year. The certification document requires vendors to confirm their eligibility to participate in the procurement by attesting to their compliance with federal and state regulations regarding debarment and suspension status.

The certification requires vendors to affirm that they and their principals are not presently debarred, suspended, proposed for debarment, or declared ineligible by any federal department or agency. Vendors must also confirm that within the three-year period preceding application, they have not been convicted of fraud or criminal offenses related to public transactions, have not violated federal or state antitrust statutes, and have not been indicted or criminally or civilly charged for enumerated offenses. Additionally, vendors must certify that they have not had any public transactions terminated for cause or default within the three-year period preceding the application. The form provides space for vendors to select either "I certify" or "I am unable to certify," with an explanation section required if unable to certify, along with signature and date fields for the vendor representative.
