About this file

This is a Statement of Work for the FR 86 Base Repair and Resurfacing Project on the Monongahela National Forest. The project scope encompasses repair of base failures and pavement failures along Forest Road 86, including milling or sawing of existing pavement, excavation of unsuitable material from failed areas, placement of rock and separation fabric to create French drains where needed, placement and compaction of aggregate base stone, and placement of Hot Mix Asphalt (HMA) base material to restore original road grade. The work also includes HMA leveling and wearing courses across the entire project area, shoulder scarification with topsoil and vegetation removal, and placement of compacted aggregate to create one-foot shoulders. Milled HMA material will be hauled to FR 101 and placed in approximately three-inch-deep compacted lifts across the full road width in areas designated by the Contracting Officer's Representative (COR).

The contractor must provide all supervision, labor, equipment, materials, quality control, and incidental services for project completion. All work shall conform to contract drawings, detailed scope specifications, industry standards, and the WVDOH 2023 Standard Specifications for Construction of Roads and Bridges, Supplemental Project Specifications (SPS). The contractor is required to submit an approved WVDOH Job Mix Formula for each HMA material, along with seven additional submittals due to the COR at minimum seven days prior to work initiation: traffic control plan, spill prevention plan, Erosion and Sediment control plan, quality control plan (including testing data for HMA products, stone, and compaction with weekly reporting), and any other submittals required by applicable contract documents. The project was initiated due to extensive flood damage from 2015 to present and heavy equipment use on this single-lane paved roadway serving the Williams River area for recreational and commuting purposes.
