About this file

This is a Respondent Questionnaire attachment for Request for Proposal No. 541209 issued by the City of Detroit Office of Contracting and Procurement on behalf of the Housing and Revitalization Department. The RFP seeks proposals from qualified underwriting firms to provide financial underwriting services for the Section 108 Loan Program, a CDBG grant-funded initiative. The selected firm will review applications submitted to the program using predetermined underwriting standards and will provide these services for a one-year contract term. The questionnaire requires respondents to provide comprehensive organizational and experience information to demonstrate their qualifications and capacity to perform the required services.

The Respondent Questionnaire is structured in two main parts: Part 1 requests respondent experience and organization details, including contact information, business background, at least three prior relevant experiences from the last five years with client references, and information on any proposed subcontractors along with their relevant experience. Part 2 addresses respondent capacity and staffing requirements, requiring submission of an organizational chart identifying key personnel, individual resumes for all key personnel, documentation of the number of employees who can be dedicated to the contract, and evidence of employee experience. The questionnaire also requires respondents to disclose any claims, lawsuits, or contract terminations within the last ten years and to verify that any proposed subcontractors are not listed on the City of Detroit's debarred and suspended suppliers list.
