About this file
This is an Appendix A Required Forms List document for the Grosse Pointe Public School System's Structured Cabling Request for Proposal (RFP), prepared by Plante Moran. The RFP solicits bids for structured cabling installation and infrastructure across seven school buildings: Brownell Middle School, Maire Elementary School, North High School, Richard Elementary School, Parcells Middle School, South High School, and Monteith Elementary School. The project scope includes installation of Category 6 cabling systems, patch panels, backbone fiber cabling, network closets, and related infrastructure components. Bid quantities vary by building, with total requirements including 342 audio/visual data cables, 582 wireless access point cables, 1,153 general data cables, and additional drop allocations across all locations. The project encompasses removal and demolition of existing infrastructure at 19 IDF closets and 2 MDF closets, reinstallation of 326 wireless access points, and creation of 6 new network closets. All cabling systems must include a certified 15-year performance guarantee, and vendors must demonstrate Category 6 certification capability and appropriate bonding and insurance limits.
Bidders must submit all required forms in Microsoft Excel format, including a Proposal Summary Form, Non-Familial Relationship Affidavit, School Safety Affidavit, Comply Exception Form, Iran Economic Sanctions Act Form, Vendor Questionnaire, equipment specifications, bid quantities, and cost schedules. The cost structure requires line-item pricing for base bid costs per building, performance bond percentages, and unit costs for various add/alternate options including Category 6A cabling upgrades, raceway installations, patch panel variations, backbone cabling, and removal/demolition services. Vendors must confirm compliance with all RFP terms and conditions, provide references with three recent projects, demonstrate workforce capacity to complete the project on an accelerated schedule (two shifts daily, six days per week from July 15 through August 15), and indicate ability to hold pricing through 2023 for future phase additions or provide guaranteed percentage discounts off educational list pricing for hardware or cabling costs.
