PURPOSE AND OVERVIEW

Multnomah County provides and funds a range of services to immigrant and refugee communities. COVID-19 has exposed systemic barriers faced by these communities in obtaining access to services. While individual departments and respective programs engage in regular outreach and service provision in immigrant and refugee communities, the pandemic highlighted opportunities for a more robust and centralized structure in addressing systemic issues to:

● provide culturally responsive and linguistically appropriate messaging and materials;

● ensure access and retention to services;

● identify disparities in outcomes;

● initiate effective communication channels;

Description

● coordinate messaging and services; and

● center community in policies and practices that impact immigrant and refugee communities.

Multnomah County (The County) seeks responses from organizations or consultants who are interested in performing an assessment and establishing a list of recommendations to ensure equitable and effective County services for immigrant and refugee communities in Multnomah County. The assessment should identify the current services specifically designed to serve immigrants and refugees through or funded by The County;

distinguish between the County’s role in supporting immigrants and refugees and the role of the refugee resettlement agencies and other service providers; and assess the actual accessibility and availability of services and supports through Multnomah County and what cultural, linguistic and systemic barriers still exist for our immigrant and refugee communities. Furthermore, it will be essential to address and codify how crises exacerbate the barriers that persist in immigrant and refugee communities in relation to services provided by or contracted out by The County. This assessment should also intentionally highlight the service gaps around intersectionality for immigrant and refugee communities, and how community members who are living with intersecting marginalized identities experience county services. Upon completion of an assessment we are requesting that the recommendations made include information gathered about effective strategies and structures that exist in other jurisdictions to improve connection and engagement with immigrant and refugee communities.

Recommendations based on the information gathered from The County and the community as a result of this (RIP) will better equip us, as The County, to serve our growing immigrant and refugee communities. How our services intersect with communities must be informed by and designed with intention around the variety of cultures, traditions, lived experience, customs, languages and unique challenges that encompass them. The assessment of gaps, barriers, the impacts of displacement and successes in current services will be examined through engaging immigrant and refugee communities and trusting the lived experience shared. This will allow us to ensure representation of individuals with lived experience in immigrant and refugee communities as we better ground and define our strategy from policy change to service access and outcomes, to community engagement.

Desired Qualifications

1. Experience with community led processes

2. Experience with community needs assessments or research activities

3. Experience in qualitative and quantitative data collection and analysis

SCOPE OF SERVICES:

We as The County have more work to do to ensure equitable access to services as well as identifying opportunities to expand or rework our services to better serve the unique and complex experiences of those who have been historically and continue to be left out of the service models. This project will map, illustrate and make recommendations to strengthen County services to immigrant and refugee communities; distinguish between the County’s role in supporting immigrants and refugees and the role of the refugee resettlement agencies and other service providers; and assess the actual accessibility and availability of services and supports through Multnomah County and what cultural, linguistic and systemic barriers still exist for our immigrant and refugee communities. This will include community engagement, a review of other jurisdictions' efforts, and a final report with actionable recommendations.

Multnomah County Health Department recognizes gaps exist for the immigrant and refugee communities and wants to identify organizations or consultants with the capacity and expertise to perform a countywide mapping assessment and analysis of the experience of our services from the variety of vantage points within our immigrant and refugee communities. By countywide, we are looking at services and supports provided by The County or funded through a contractual agreement by The County to perform such services in immigrant and refugee communities. The initial assessment and analysis should include data collection and stakeholder engagement. This could look like data and information collection within The County, with contracted providers funded by The County to do work in immigrant and refugee communities, with community based organizations and groups who are deeply connected in immigrant and refugee communities within Multnomah County, and with community members with lived experience as an immigrant or refugee. Specifically, quantitative and qualitative data should be included in the analysis. Data collection may include, but may not be limited to assessing service utilization and outcomes, along with barriers along the social determinants of health identified by intended service users. The analysis will include sense-making of the data and what it is telling us, along with recommendations to address the challenges and barriers identified in the data collection efforts. Data dimensions within the analysis should include an understanding of where services are geographically located, where the majority of participants live, as well as the distance from these services to the areas in our county where we know we have large populations of immigrant and refugee communities. Additionally, the analysis should compare who is being referred to services countywide and if there are disparities or disproportionalities present and a comparison of the demographics of the immigrant and refugee communities that we serve and the staff in our programs.

The County is seeking information about organizations or consultants who have a deep understanding of the unique experiences different immigrant and refugee communities might hold as well as the intersectionalities that exist for people but that most services are silent to. This work will require an organization or consultant to have demonstrated experience honoring the experience of immigrant and refugee communities and the well-placed distrust in our system, organization and services - while making all efforts to ensure we are centering community voice with every effort towards the change needed to ensure equity for all of our community members, not just some. Organizations and consultants interested in this opportunity will have demonstrated experience working on similar assessments or analysis with government agencies, community-based organizations and within immigrant and refugee communities. Organizations and consultants must have relationships and trust built in communities The County aims to impact through this analysis and recommendations.

DEFINITIONS

Culturally Responsive Services: Cultural responsive services are general services that have been adapted to honor and align with the beliefs, practices, culture and linguistic needs of diverse consumer/client populations and communities whose members identify as having particular cultural or linguistic affiliations by virtue of their place of birth, ancestry or ethnic origin, religion, preferred language or language spoken at home. Culturally responsive services also refer to services provided in a way that is culturally responsive to the varied and intersecting “biological, social and cultural categories such as gender identity, class, ability, sexual orientation, religion, caste, and other axes of identity” (Wikipedia, 2017). Culturally responsive organizations typically refer to organizations that possess the knowledge and capacity to respond to the issues of diverse, multicultural communities at multiple intervention points. Culturally responsive organizations affirmatively adopt and integrate the cultural and social norms and practices of the communities they serve. These agencies seek to comprehensively address internal power and privilege dynamics throughout their service delivery, personnel practices and leadership structure. (Powell, 2015, as cited in Multnomah County Office of Diversity and Equity, Contracting and Procurement for Culturally Specific and Responsive Services, 2017).

Culturally Specific Services: Culturally specific services are services provided for specific populations based on their particular needs, where the majority of members/clients are reflective of that community, and use language, structures and settings familiar to the culture of the target population to create an environment of belonging and safety in which services are delivered. Culturally specific organizations typically refer to organizations with a majority of members/clients from a particular community. Culturally specific organizations also have a culturally focused organizational identity and environment, a positive track record of successful community engagement, and recognition from the community served as advancing the best interests of that community.

(Curry-Stevens, A., Reyes, M.E. and Coalition of Communities of Color, 2014).

Immigrant and Refugee Communities: For the purposes of this project, immigrant and refugee communities refers to individuals who are displaced, and in general, have left their home countries due to economic opportunities, family reunification. war, environmental or economic disasters, political persecution, and/or religious or ethnic intolerance.

There are legal terms and status associated with immigration status, and asylum seeker or refugee status.

Linguistically Appropriate: Linguistically appropriate services are respectful of and responsive to the health beliefs, practices and needs of diverse patients/clients. By tailoring services to an individual's culture and language preference, health professionals can help bring about positive health outcomes for diverse populations, (US Department of Health and Human Services, Office of Minority Health, 2021).

Intersectionality: the interconnected nature of social categorizations such as race, class, and gender as they apply to a given individual or group, regarded as creating overlapping and interdependent systems of discrimination or disadvantage (Oxford Dictionary, 2021).

Community based participatory research (CBPR): Community-based participatory research (CBPR) is a partnership approach to research that equitably involves community members, organizational representatives, researchers, and others in all aspects of the research process, with all partners in the process contributing expertise and sharing in the decision-making and ownership (Wikipedia, 2021).

Needs Assessments: A needs assessment is triggered by a need to better understand a particular situation and the conditions faced by populations of concern, whether in the context of a response to a sudden crisis or an ongoing planning effort during a protracted crisis (UNHCR, 2017).

Participatory Action Research (PAR): Participatory action research (PAR) is an approach to action research emphasizing participation and action by members of communities affected by that research (Wikipedia, 2021).
