About this file
File Summary
This document comprises the final design plans for Washington County's NW Turk Road Bridge Replacement project (Project Number 100762), prepared by Kittelson & Associates, Inc. with geotechnical engineering by Central Engineering Services. The project involves the replacement of the existing timber bridge structure (#671374) over West Fork Dairy Creek with a new 75-foot precast prestressed concrete slab beam bridge. The scope of work includes bridge construction, roadway approaches spanning 370 feet from station 1+13.00 to station 4+81.50, installation of guardrails, drainage improvements, erosion and sediment control measures, driveway approaches, and sheet pile retaining walls. The design incorporates six 30-inch precast prestressed slab beams supported on two bents with pipe pile foundations driven to nominal resistance depths of approximately 150 to 151 feet. Construction activities are limited during sensitive environmental periods, with in-water work restricted to July 15 through September 30. The project is estimated to cost between $1,000,000 and $2,000,000 and will require temporary road closure with detour routes to maintain local access to affected properties.
The design plans specify asphalt concrete pavement with 2-inch wearing course and base course over an 8-inch aggregate base course on both travel lanes. Erosion control measures include 18-inch compost filter socks totaling 910 linear feet, slope matting coverage of 420 square yards, permanent seed mix over 460 square yards of disturbed areas, and six check dams. Sheet pile retaining walls utilizing ASTM A328 SKZ24 sections will be installed on both sides of the structure, with Class 200 riprap slope protection and basin details specified. Bridge components include 2-tube side mount bridge rail systems, drainage curbs, three driveway approaches with asphalt or aggregate surfacing, and permanent signing. The project design complies with Oregon standards including utility notification requirements under ORS 92.044 and OAR 952-001-0010 through 0090, with coordination required for existing utility relocations involving Ziply telecommunications conduit and PGE power lines.

1. IF VEGETATIVE SEED MIXES ARE SPECIFIED, SEEDING MUST TAKE PLACE NO LATER THAN OCTOBER 1ST. THE TYPE AND PERCENTAGES OF SEED IN THE MIX MUST BE IDENTIFIED ON THE PLANS.
2. ALL PUMPING OF SEDIMENT LADEN WATER SHALL BE DISCHARGED OVER AN UNDISTURBED,
PREFERABLY VEGETATED AREA, AND THROUGH A SEDIMENT CONTROL BMP (I.E. FILTER BAG).
3. ALL EXPOSED SOILS MUST BE COVERED DURING THE WET WEATHER PERIOD, OCTOBER 1ST - MAY
31ST.
4. THE IN-WATER WORK PERIOD FOR THIS PROJECT ON WEST FORK DAIRY CREEK IS JULY 15TH -
SEPTEMBER 30TH.
LOCAL AGENCY-SPECIFIC EROSION CONTROL NOTES
1. ALL PERIMETER CONTROL BMPS (INLET PROTECTION, SEDIMENT FENCING, TEMPORARY CONSTRUCTION WORK ZONE FENCING, SEDIMENT BARRIERS, AND GRAVEL CONSTRUCTION ENTRANCES, ETC.) MUST BE IN PLACE, FUNCTIONAL, AND APPROVED IN AN INITIAL INSPECTION, PRIOR TO COMMENCEMENT OF CONSTRUCTION
ACTIVITIES.
2. SEDIMENT BARRIERS APPROVED FOR USE INCLUDE SEDIMENT FENCE, WATTLES, AND COMPOST SOCK.
3. SENSITIVE RESOURCES INCLUDING, BUT NOT LIMITED TO, TREES, WETLANDS, AND VEGETATED CORRIDORS SHALL BE CLEARLY DELINEATED WITH TEMPORARY CONSTRUCTION WORK ZONE FENCING (ORANGE COLOR) IN A MANNER THAN IS CLEARLY VISIBLE TO ANYONE IN THE AREA. NO ACTIVITIES ARE PERMITTED TO OCCUR
BEYOND THE CONSTRUCTION BARRIER.
4. ADDITIONAL MEASURES INCLUDING, BUT NOT LIMITED TO, STREET SWEEPING AND TIRE WASHING, MAY BE REQUIRED TO INSURE THAT ALL PAVED AREAS ARE KEPT CLEAN FOR THE DURATION OF THE PROJECT.
PRE-CONSTRUCTION, CLEARING, AND DEMOLITION NOTES
1. SLOPE TO RECEIVE TEMPORARY OR PERMANENT SEEDING SHALL HAVE THE SURFACE ROUGHENED BY MEANS OF TRACK-WALKING OR THE USE OF OTHER APPROVED IMPLEMENTS. SURFACE ROUGHENING IMPROVES SEED BEDDING AND
REDUCES RUN-OFF VELOCITY.
2. LONG TERM SLOPE STABILIZATION MEASURES SHALL INCLUDE THE ESTABLISHMENT OF PERMANENT VEGETATIVE COVER VIA
SEEDING WITH APPROVED MIX AND APPLICATION RATE.
3. TEMPORARY SLOPE STABILIZATION MEASURES SHALL INCLUDE: COVERING EXISTING EXPOSED SOIL WITH PLASTIC
SHEETING, STRAW MULCHING, WOOD CHIPS, OR OTHER APPROVED MEASURES.
4. STOCKPILED SOIL OR STRIPPINGS SHALL BE PLACED IN A STABLE LOCATION AND CONFIGURATION. DURING "WET WEATHER" PERIODS, STOCKPILES SHALL BE COVERED WITH PLASTIC SHEETING OR STRAW MULCH. SEDIMENT FENCE IS REQUIRED
AROUND THE PERIMETER OF THE STOCKPILE.
5. EXPOSED CUT OR FILL AREAS SHALL BE STABILIZED THROUGH THE USE OF TEMPORARY SEEDING AND MULCHING, EROSION CONTROL BLANKETS OR MATS, MID-SLOPE SEDIMENT FENCES OR WATTLES, OR OTHER APPROPRIATE MEASURES. SLOPES
EXCEEDING 25% MAY REQUIRE ADDITIONAL EROSION CONTROL MEASURES.
6. AREAS SUBJECT TO WIND EROSION SHALL USE APPROPRIATE DUST CONTROL MEASURES INCLUDING THE APPLICATION OF A FINE SPRAY OF WATER, PLASTIC SHEETING, STRAW MULCHING, OR OTHER APPROVED MEASURES.
7. CONSTRUCTION VEHICLES AND EQUIPMENT SHALL ENTER THE WORK AREA FROM EXISTING GRAVEL AREAS. ADDITIONAL MEASURES INCLUDING, BUT NOT LIMITED TO, TIRE WASHING, MAY BE REQUIRED TO INSURE THAT ALL PAVED AREAS ARE
KEPT CLEAN FOR THE DURATION OF THE PROJECT.
8. SATURATED MATERIALS THAT ARE HAULED OFF-SITE MUST BE TRANSPORTED IN WATER-TIGHT TRUCKS TO ELIMINATE
SPILLAGE OF SEDIMENT AND SEDIMENT-LADEN WATER.
9. AN AREA SHALL BE PROVIDED FOR THE WASHING OUT OF CONCRETE TRUCKS IN A LOCATION THAT DOES NOT PROVIDE RUN-OFF THAT CAN ENTER THE STORM WATER SYSTEM OR STREAMS/WETLANDS AND OTHER SENSITIVE AREAS. IF THE CONCRETE WASH-OUT AREA CANNOT BE CONSTRUCTED GREATER THAN 50' FROM ANY DISCHARGE POINT, SECONDARY MEASURES SUCH AS BERMS OR TEMPORARY SETTLING PITS MAY BE REQUIRED. THE WASH-OUT SHALL BE LOCATED WITHIN
SIX FEET OF TRUCK ACCESS AND BE CLEANED WHEN IT REACHES 50% OF THE CAPACITY.
10. SWEEPINGS FROM EXPOSED AGGREGATE CONCRETE SHALL NOT BE TRANSFERRED TO THE STORM WATER SYSTEM.
SWEEPINGS SHALL BE PICKED UP AND DISPOSED IN THE TRASH.
11. USE BMPs SUCH AS CHECK-DAMS, BERMS, AND INLET PROTECTION TO PREVENT RUN-OFF FROM REACHING DISCHARGE
POINTS.

GENERAL NOTES
1. Elevations are based on National Geodetic Vertical Datum (NGVD) 29.
2. 1' contour intervals shown.
3. Geotechnical data shown on this drawing are a consolidation and/or revision in terminology from the drill logs. The drill logs used in compiling this drawing are available upon request. Contractor shall refer to geotechnical reports and drill logs and information therein.
4. The number of blows required to drive the sampler of three, 6-inch increments of penetration were recorded in the field. The sum of the blow counts for the last two 6-inch increments of penetration are reported as the ASTM Test Method D 1556 Standard Penetration Test (SPT) N-value. Samples were collected using a 1-inch inside diameter standard split spoon sampler.

GENERAL FOUNDATION NOTES:
All Bents, Provide PP18.00 x 0.625, ASTM A252, Grade 3 piling driven open-ended to a nominal resistance as shown in the Foundation Table.
Pile tip elevation for minimum pile penetration at all bents is shown as according to the Foundation Table. Drive piles to the specified nominal resistance using driving criteria developed from the Wave Equation Analysis and verified using a Pile Driving Analyzer and Capwap Analysis as specified in the Special Provisions.
Conduct PDA testing during pile driving and at restrike.
All bents are parallel.
