About this file

This is an Identification of Subcontractor/Supplier/Subconsultant Form (ISF) issued by the Cook County Office of the Chief Procurement Officer for the LiDAR Data Acquisition project (RFP 2610-01230). The form serves as a mandatory submission requirement for all bids, proposals, and qualifications, requiring contractors to fully disclose and document each subcontractor, supplier, or subconsultant that will be utilized on the contract. Contractors must complete a separate ISF for each subcontractor or supplier and submit updated forms if any changes occur in subcontractor utilization during the contract term. The form requires identification of authorized contacts, detailed company information, service descriptions, subcontract pricing, and estimated project timelines for all parties involved.

The ISF establishes that all subcontract documents must incorporate the requirements of the prime contract as applicable and that subcontractors may not use contract participation to hinder progress on other concurrent projects. The form explicitly states that submission of the ISF does not approve any changes to the contract's approved Minority and Women Business Enterprise (MBE/WBE) Utilization Plan, with any modifications requiring separate submission to the Office of the Contract Compliance. The prime contractor retains full responsibility and liability for subcontractor performance regardless of ISF submission, and the Cook County Office of the Chief Procurement Officer reserves the right to request copies of all written subcontractor agreements upon demand.
