About this file

This is an Ownership Disclosure Certification form issued by the New Jersey Department of the Treasury, Division of Purchase and Property, required for vendor participation in Atlantic Cape Community College's BID 1881 for Building S Stairwell and Carpet Renovations. The project solicits sealed bids for two separate trade contracts: Trade 1 (Drywall & Painting) and Trade 2 (Carpet & Flooring), with bidders permitted to submit proposals for one or both trades. Bids must be submitted in sealed envelopes clearly marked with the bidder's name and applicable trade to the Atlantic Cape Community College Purchasing Department, Room J202, 5100 Black Horse Pike, Mays Landing, New Jersey 08330, by April 28, 2026 at 11:00 AM EST, at which time bids will be publicly opened and read aloud. Each trade will be evaluated and awarded separately in accordance with New Jersey Community College Contract Law.

All bidders must comply with statutory requirements including Affirmative Action Requirements (N.J.S.A. 10:5-31 et seq.), Ownership Disclosure (N.J.S.A. 52:25-24.2), New Jersey Business Registration (N.J.S.A. 52:32-44), the New Jersey Prevailing Wage Act, and the Public Works Contractor Registration Act. Vendors must complete the Ownership Disclosure Form by providing names and addresses of all stockholders, partners, or members owning 10% or greater interest in the vendor entity, with continued disclosure of any parent entities meeting the same ownership threshold. Non-profit entities and sole proprietors are exempt from disclosure requirements. Alternatively, vendors with publicly traded parent entities may submit the parent company information and Securities and Exchange Commission filing references instead of completing the full disclosure form.
