About this file

This is a Statement of Work for a construction project under Public Law 86-121 administered by the Indian Health Service Portland Area Office. The project involves installation of individual septic and water pressure systems on Fee land in Sequim, Washington (130 Red Hawk Road, Clallam County) for homeowner Collin Haupt, a member of the Makah Tribe. The project number is PO-22-D05F with a completion timeline of 30 calendar days.

The septic system scope includes installing a 1,000-gallon two-compartment concrete septic tank with effluent filter, a 1,000-gallon pump chamber with 0.5 HP effluent pump (Orenco model PF100511 or equivalent), 4-inch ASTM D3034 PVC gravity sewage transport piping, and approximately 750 square feet of subsurface drip dispersal area using Netafilm Bioline dripline. The water pressure system requires installation of a 3/4 HP submersible pump (Goulds 10G07 or equivalent) in an existing well at 162 feet below ground surface, approximately 155 linear feet of 1-inch SCH 40 PVC water service line to the house, a variable frequency drive (Aquavar SOLO2 or equivalent) in a standalone structure, a 2-gallon pressure tank (Amtrol Well X-Trol WX-101 or equivalent) in an underground valve vault, and approximately 200 feet of electrical conduit to the home subpanel. Work must comply with Washington Administrative Code standards for on-site sewage systems, wells, and plumbing codes. The contractor is responsible for pre-construction submittals of manufacturer specifications, site utility identification, debris removal, finish grading, and coordination with the Contracting Officer's Representative (Madeline Keep, 360-536-3364) for required inspections and approvals. Upon completion, all facilities transfer to the homeowner for operation and maintenance.

The proposed septic system is to consist of 4-inch ASTM D3034 PVC gravity sewage transport pipe, an underground doublesweep cleanout, a 1,000-gallon two-compartment concrete septic tank with effluent filter, a 1,000 gallon pump chamber with effluent pump (Orenco model PF100511 or equivalent, 0.5 HP) and controls, ¾-inch Schedule 40 PVC pressure supply and flush lines, continuous flush headworks, 1-inch air/vacuum release valves, and subsurface Netafilm Bioline dripline with approximately 750 square feet of dispersal area per County-approved design (Attachment: County Approved Septic Design – Haupt) and local, state, and Tribal regulations. The contractor shall remove the temporary well cap and appurtenances currently on the well, install a well pump, drop pipe, submersible cable, pitless adapter, metal well cap, approximately 155 linear feet of 1” SCH 40 PVC water line from the well to the existing home water stub-out, install a variable frequency drive (Aquavar SOLO2 or equivalent) in a standalone structure adjacent to the well head, install a 2-gallon pressure tank (Amtrol Well X-Trol WX-101 or equivalent) inside of an underground valve vault adjacent to the control panel structure, install buried electrical conduit to the home subpanel, and complete well and pressure system disinfection. The existing well log, pump test data, and list of existing well appurtenances for the home are provided (see Attachments). Further investigation of the site before and after bidding is the responsibility of the Contractor. Any permits are the responsibility of the Contractor.

B. Ownership

This project will provide sanitation facilities to a private home owned by a member of the Makah Tribe. Upon completion, the facilities will be transferred to the homeowner. The system will be operated by the homeowner.
