About this file
This is a Certification Regarding Debarment, Suspension, and Other Responsibility Matters form issued by the Los Alamos County Procurement Division in New Mexico for use in responding to the Employee Vision Benefits solicitation. The form requires offerors to certify their eligibility and responsibility status as a condition of bid submission and contract award consideration.
The certification requires offerors to attest that neither they nor their principals are currently debarred, suspended, proposed for debarment, or excluded from covered transactions by any federal, state, or local agency. Offerors must also confirm that within the preceding three-year period they have not been convicted of fraud, criminal offenses related to public transactions, antitrust violations, embezzlement, theft, forgery, bribery, falsification of records, or crimes indicating lack of business integrity. Additionally, offerors must certify they have not violated contract provisions through deliberate failure to perform, had unsatisfactory performance records, violated ethical standards under the County Procurement Code, are not current or former County employees or immediate family members of County employees or officials, and have not had public contracts terminated for cause or default. Any offeror unable to certify these statements must provide written explanation. Falsification of any statement in the certification constitutes grounds for non-consideration or contract award rescission.
