About this file
This Highway Occupancy Permit, issued by the Pennsylvania Department of Transportation (PennDOT) District 4-0 office in Lackawanna County, authorizes Gentex Corporation to install a low-volume driveway on State Route 171 in Fell Township. The permit (No. 04074583) was issued on June 24, 2026, and expires on June 24, 2027, covering work along a 200-foot section of the highway right-of-way (from offset 1060 to 1260). The permittee is required to contact the district permit office at least three work days prior to commencing work, before any paving or curb installation, and before final restoration activities. An inspector will be assigned on a spot-inspection basis, with all inspection costs charged to the permittee. The permit was completed and acknowledged by Jonathan A. Eboli, P.E., District Executive, and Michael B. Carroll, Secretary of Transportation.
The permit carries a $50 filing fee and imposes extensive conditions on the permittee, including requirements to maintain minimum sight distance, comply with PennDOT roadway standards for curbs and shoulders, prevent surface drainage onto the highway right-of-way, maintain proper vertical clearance, and ensure traffic control measures in accordance with PennDOT publications. The permittee is responsible for restoring all disturbed areas, removing mud and debris from vehicles before entering the highway, identifying and protecting any PennDOT-owned facilities within the work limits, and maintaining access from the pavement edge to at least 20 feet outside the right-of-way. Sidewalk construction must comply with ADA and PennDOT standards. The permittee must notify PennDOT upon work completion and maintain the facility in accordance with all applicable regulations and restrictions prescribed by the Pennsylvania Department of Transportation.
