About this file
This is an Identification of Subcontractor/Supplier/Subconsultant Form (ISF) issued by the Cook County Office of the Chief Procurement Officer for use in state and local contract opportunities in Illinois. The form is a mandatory disclosure document that contractors must complete and submit with each bid, Request for Proposal, or Request for Qualification. Contractors are required to identify all subcontractors, suppliers, and subconsultants that will be utilized on the contract and must submit an updated ISF if any changes occur in the utilization of these parties during contract performance. The form requests comprehensive information including authorized contacts, company addresses, telephone and fax numbers, estimated start and completion dates, and a detailed description of services or supplies to be provided along with the total price of each subcontract.
The ISF form emphasizes that all subcontract documents must incorporate the requirements of the awarded contract and that subcontractors shall not be hindered from maintaining progress on other contracts. The form explicitly states that the prime contractor retains full responsibility for organization, performance, and quality of work and is not relieved of any obligations by the submission of this form. Any proposed changes to approved Minority Business Enterprise (MBE) or Women Business Enterprise (WBE) utilization plans require separate submission to the Office of Contract Compliance and are not approved through the ISF alone. The form includes a section for OCPO office use only for contract number verification, disqualification status, and completion checks.
